目的:探讨基于扩散峰度成像(Diffusion kurtosis imaging,DKI)的瘤内和瘤周定量信息在鉴别不同HER-2 表达乳腺癌中的可行性.方法:回顾性分析 2019年12 月—2023 年12 月经术后病理证实的150例浸润性乳腺癌女性患者(单侧单发)的资料,依据HER-2表达分为过表达、低表达和零表达3组.患者术前均完成乳腺DKI检查.选取病灶实性成分最大层面,分别在瘤内、距离肿瘤轮廓外 2mm及 5mm处手动勾画感兴趣区定义为ROI1、ROI2 和ROI3,记录 3 处的平均扩散峰度(MK)及平均扩散率(MD)的最大值、最小值和平均值.采用Kruskal-Wallis检验和单因素方差分析比较组间的瘤内、瘤周MK及MD值.通过受试者工作特征(ROC)曲线和Delong检验分析各参数及联合模型的诊断效能,P<0.05 为差异有统计学意义.结果:ROI1参数显示:零表达组的MDmax值和MDmean值均最高,依次高于低表达组和过表达组(P=0.010、0.019).ROI2 参数显示:零表达组的MDmax和MDmean值均最高,依次高于低表达组和过表达组(P=0.023、0.001).而零表达组的MKmean值最低,依次低于低表达组和过表达组(P=0.034).ROI3 参数显示:零表达组的MDmax值最高,依次高于低表达组和过表达组(P=0.029).基于ROI1、ROI2和ROI3 测得的定量参数值诊断零表达和过表达两组乳腺癌的AUC分别为 0.623、0.714、0.659,ROI1 联合ROI2、ROI1 联合ROI3 以及三者联合鉴别零表达和过表达两组乳腺癌的AUC分别为 0.768、0.696 和 0.767,均高于单一模型的诊断效能(P<0.05).结论:瘤内、瘤周DKI弥散信息有助于鉴别不同HER-2 表达的浸润性乳腺癌,可为乳腺癌内分泌治疗前提供影像诊断新依据.

Objective:To investigate the feasibility of using quantitative intratumoral and peritumoral parameters derived from diffusion kurtosis imaging(DKI)for discriminating breast cancers with different HER-2 expression levels.Methods:A ret-rospective analysis was performed on 150 female patients with pathologically confirmed unilateral solitary invasive breast cancer(from December 2019 to December 2023).Based on HER-2 expression,patients were divided into three groups:over-expres-sion,low-expression,and zero-expression.All patients underwent preoperative breast DKI.The largest cross-section of the sol-id tumor component was selected,and regions of interest(ROIs)were manually drawn within the tumor(ROI1),and at 2 mm(ROI2)and 5 mm(ROI3)beyond the tumor border.The maximum,minimum,and mean values of mean kurtosis(MK)and mean diffusivity(MD)were recorded for each ROI.The Kruskal-Wallis test and one-way ANOVA were used to compare intratumoral and peritumoral MK and MD values among groups.The diagnostic performance of individual parameters and combined models was evaluated using receiver operating characteristic(ROC)curves and the Delong test,with P<0.05 considered statistically sig-nificant.Results:For ROI1,the zero-expression group showed the highest MDmax and MDmean values,significantly greater than those in the low-expression and over-expression groups(P=0.010,0.019).For ROI2,the zero-expression group also had the highest MDmax and MDmean values(P=0.023,P=0.001),but the lowest MKmean value(P=0.034).In ROI3,the zero-expression group exhibited the highest MDmax value(P=0.029).The AUC values for distinguishing zero-expression from over-expression groups us-ing parameters from ROI1,ROI2 and ROI3 were 0.623,0.714 and 0.659,respectively.Combined models(ROI1+ROI2,ROI1+ROI3 and all three)yielded AUCs of 0.768,0.696 and 0.767,respectively,all significantly higher than those of single param-eters(P<0.05).Conclusion:DKI-derived diffusion parameters from both intratumoral and peritumoral regions can help differenti-ate invasive breast cancers with varying HER2 expression levels,providing new imaging evidence to guide endocrine therapy.
