原发性闭角型青光眼患者神经纤维层厚度分布特点与脉络膜毛细血管缺失的关系OA
The relationship between the distribution characteristics of retinal nerve fi-ber layer and choroidal microvasculature dropout in patients with primary angle closure glaucoma
目的 探讨原发性闭角型青光眼(PACG)患者神经纤维层厚度(RNFL)分布特点及其与脉络膜毛细血管缺失(CMvD)的关系.方法 选取2020年12月至2024年12月我院收治的76例(76眼)PACG患者,根据其脉络膜毛细血管是否缺失分为CMvD组(32眼)和非CMvD组(44眼),另取30例(30眼)同期在我院体检的健康人作为对照组.比较3组受试者临床资料、RNFL厚度及视盘参数差异;多元Logistic回归模型分析患者CMvD发生的相关因素;采用Pearson相关性分析RNFL厚度与视盘参数及视野平均缺损(MD)的相关性;采用限制性三次样条分析RNFL厚度与PACG患者CMvD的非线性关联;采用Log-binomial分析不同RNFL厚度和不同年龄CMvD发病风险;并比较各组观测值差异.结果3组受试者年龄、眼轴长度、居住地及近视人数比例比较差异均有统计学意义(均为P<0.05),且CMvD组患者年龄、农村居住及近视人数比例均高于对照组,眼轴长度均明显低于对照组(均为P<0.05).3组受试者上方、颞侧、下方、鼻侧及平均RNFL厚度比较差异均有统计学意义(均为P<0.05).CMvD组、非CMvD组患者上方、颞侧、下方、鼻侧及平均RNFL厚度和视盘周围血管密度(cpVD)均明显低于对照组,CMvD组患者上方、颞侧、下方、鼻侧、平均RNFL和cpVD均明显低于非CMvD组(均为P<0.05).3组受试者视杯容积(CV)、盘沿面积(RA)、平均杯盘比(C/D)、垂直C/D、水平C/D、视野MD比较差异均有统计学意义(均为P<0.05).CMvD组、非CMvD组患者CV、平均C/D、垂直C/D、水平C/D均明显高于对照组,RA、视野MD均明显低于对照组,且CMvD组患者CV、平均C/D、垂直C/D、水平C/D均明显高于非CMvD组,RA、视野MD均明显低于非CMvD组(均为P<0.05).Pearson相关性分析表明,患者视盘视网膜平均RNFL厚度分别与CV、平均C/D、垂直C/D、水平C/D呈负相关,与平均cpVD、RA、视野 MD均呈正相关(均为P<0.05).多元Logistic回归模型分析结果显示,年龄、平均RNFL厚度是PACG患者CMvD发生的相关因素(均为P<0.05).限制性三次样条回归分析结果表明,平均RNFL厚度与PACG患者CMvD呈明显负相关(P<0.05).Log-binomial回归分析显示,在平均RNFL厚度<81.53 μm患者中,年龄≥62岁患者CMvD患病率显著高于年龄<62岁(P<0.05).重复性一致性检验结果显示,CMvD和RNFL范围及位置重复性一致性均较好.结论 RNFL厚度与PACG患者CMvD密切相关,临床对于RNFL厚度异常的患者应重点关注,并制定个体化的干预策略提高生活质量.
Objective To explore the distribution characteristics of the retinal nerve fiber layer(RNFL)in patients with primary angle closure glaucoma(PACG)and analyze its relationship with choroidal microvasculature dropout(CM-vD).Methods Seventy-six patients(76 eyes)with PACG treated in our hospital from December 2020 to December 2024 were selected as the research subjects.According to whether the choriocapillaris were missing,the patients were divided in-to the CMvD group(32 eyes)and the non-CMvD group(44 eyes).Another 30 healthy individuals(30 eyes)who underwent physical examination in our hospital during the same period were selected as the control group.The clinical data,RNFL thickness,and optic disc parameters were compared among the three groups of patients.A multivariate logistic regression model was used to analyze the factors related to the CMvD occurrence in patients.Pearson correlation analysis was used to examine the correlation between RNFL thickness,optic disc parameters,and visual field mean deviation(MD).A restricted cubic spline was used to analyze the nonlinear association between RNFL thickness and the occurrence of CMvD in PACG patients.Log-binomial regression was used to analyze the risk of CMvD in different RNFL thicknesses and different ages.The differences in measured values among the groups were also compared.Results There were statistically significant differences in age,axial length,place of residence,and the proportion of myopic patients among the three groups(all P<0.05).The CMvD group had more patients aged,residing in rural areas,and with myopia compared to the control group,while the axial length was significantly lower than that in the control group(all P<0.05).The differences in superior,temporal,inferior,nasal,and mean RNFL thickness among the three groups were statistically significant(all P<0.05).The superior,temporal,inferior,nasal,and mean RNFL thickness,as well as the circumpapillary vessel density(cpVD)in the CMvD group and the non-CMvD group were significantly lower than those in the control group,and the superior,temporal,inferior,nasal,and mean RNFL thickness,as well as cpVD in the CMvD group were significantly lower than those in the non-CMvD group(all P<0.05).Statistically significant differences were observed in the cup volume(CV),rim area(RA),mean cup-to-disk ratio(C/D),vertical C/D,horizontal C/D,and visual field MD among the three groups(all P<0.05).The CV,mean C/D,vertical C/D,and horizontal C/D in the CMvD group and the non-CMvD group were significantly higher than those in the control group,while the RA and visual field MD were significantly lower than those in the control group;and the CV,mean C/D,vertical C/D,and horizontal C/D in the CMvD group were significantly higher than those in the non-CMvD group,while the RA and visual field MD were significantly lower than those in the non-CMvD group(all P<0.05).Pearson correlation analysis showed that the mean RNFL thickness in the optic disc and retina of patients was negatively correlated with the CV,mean C/D,vertical C/D,and horizontal C/D,and positively correlated with mean cpVD,RA,and visual field MD(all P<0.05).Multivariate logistic regression analysis showed that age and mean RNFL thickness were related factors for the occurrence of CMvD in PACG patients(both P<0.05).The restricted cubic spline regression analysis showed that mean RNFL thickness was significantly negatively correlated with the occurrence of CMvD in PACG patients(P<0.05).Log-bino-mial regression analysis showed that in patients with a mean RNFL thickness<81.53 μm,the incidence of CMvD in patients aged ≥ 62 years was significantly higher than that in patients aged<62 years(P<0.05).The repeatability and consistency test results showed that CMvD and RNFL had good repeatability and consistency in range and position.Conclusion The thickness of RNFL is closely related to the occurrence of CMvD in PACG patients.Clinical attention should be focused on patients with abnormal RNFL thickness,and personalized intervention strategies should be developed to improve their quali-ty of life.
