目的 评估免疫治疗、放射治疗、化疗和靶向治疗在晚期转移性食管癌患者中作为一线治疗方案的疗效.方法 通过对PubMed、Embase和Cochrane Library数据库进行系统检索,收集和分析关于晚期转移性食管癌不同治疗方案.采用RevMan 5.4 软件中的Cochrane偏倚风险评估工具来评估纳入文献的质量,并使用RStudio及RevMan 5.4 软件进行网状Meta分析,以综合评估不同治疗方案的疗效.本研究运用贝叶斯统计技术进行频率分析,从而完成了相应的网状Meta分析.结果 共纳入 60 项研究,24 681 例患者,42 种不同的治疗方案;其中有27 项随机对照试验,包含 9 730 例患者.结果 表明,免疫联合化疗的组合排名较好,其中"度伐利尤单抗+替西木单抗+紫杉醇+顺铂"方案、"卡瑞利珠单抗+紫杉醇+顺铂"方案以及"特瑞普利单抗+紫杉醇+顺铂"方案均取得了较好的PFS优势.结论 在晚期转移性食管癌病人中,相较于传统化疗和化疗联合靶向治疗,采用免疫疗法配合化疗方案在PFS上可获得更优的结果.

Objective To evaluate the efficacy of immunotherapy,radiotherapy,chemotherapy,and targeted therapy as first-line treatment regimens in patients with advanced metastatic esophageal cancer.Methods By systematically searching multiple databases such as PubMed,Embase,and Cochrane Library,information on different treatment options for advanced metastatic esophageal cancer was collected and analyzed.Cochrane risk-of-bias tool(RevMan 5.4)was employed to evaluate the quality of the included literature,and RStudio and RevMan 5.4 software were conducted to a network meta-analysis,to comprehensively evaluate the efficacy of different treatment regimens.This study used Bayesian model for frequency analysis,to complete the corresponding network meta-analysis.Results A total of 60 studies were included,involving 24 681 patients and 42 different treatment options.Among them,there are 27 randomized controlled trials involving 9 730 patients.The results showed that the combination of immunotherapy and chemotherapy ranked well,among which the"durvalumab+tremelimumab+paclitaxel+cisplatin"regimen,"camrelizumab+paclitaxel+cisplatin"regimen,and"toripalimab+paclitaxel+cisplatin"regimen all achieved good progression-free survival advantages.Conclusion In patients with advanced metastatic esophageal cancer,immunotherapy combined with chemotherapy yielded superior progression-free survival compared to both conventional chemotherapy and chemotherapy combined with targeted therapy.
