Intraoperative cell salvage(IOCS)is a key blood conservation strategy.Its application in liver transplantation for hepatocellular carcinoma,however,has attracted considerable attention due to oncological concerns.However,concerns persist that IOCS may increase the risk of tumor recurrence due to the potential presence of circulating tumor cells in salvaged blood.This article systematically reviews studies from the past five years regarding the oncological safety of IOCS in liver trans-plantation for hepatocellular carcinoma.A synthesis of evidence from multiple meta-analyses and retrospective studies indicates that IOCS does not significantly increase the overall risk of tumor recurrence or adversely affect long-term patient survival.Ad-ditionally,some evidence indicates that the safety profile of IOCS may be associated with individual patient characteristics,such as pre-transplant alpha-fetoprotein levels,which offers a foundation for risk stratification.This review suggests that for carefully selected patients undergoing liver transplantation for hepatocellular carcinoma,IOCS appears to be a safe and effective option for blood conservation.Future clinical decisions and research should focus on developing integrated risk prediction mod-els and identifying key protective measures to achieve the precision application and safety management of this technique.
