从络病理论辨治女性慢性盆腔疼痛OA

Female chronic pelvic pain(CPP)is a common gynecological condition characterized by a prolonged disease course and recurrent episodes,severely impairing patients' quality of life.Guided by the theory of collateral disease,and based on YE Tianshi's principle that"in the early stage,qi stagnates in the channels,and in the chronic stage,blood injury enters the collaterals",this study explores the etiology,pathogenesis,and differentiation-based treatment of female CPP.It is proposed that the pathological evolution of CPP corresponds to the progression of"col-lateral qi stagnation,blood stasis obstructing the collaterals,and deficiency of collaterals due to malnourishment".Accordingly,treatment should follow the course of disease progression,applying the stage-based therapeutic princi-ples of"regulating qi and promoting blood circulation,resolving stasis and unblocking collaterals,and nourishing col-laterals to relieve pain".Thus,a three-stage system of"three pathological stages and three corresponding therapeutic approaches"is established.Clinical practice has demonstrated that this system effectively alleviates symptoms,re-duces recurrence rates,and improves reproductive outcomes.It provides a novel theoretical framework and therapeu-tic approach for the traditional Chinese medicine management of female chronic pelvic pain.
