间质性肺病(ILD)是一类以弥漫性肺泡炎以及肺间质纤维化为主要病理表现的疾病总称.骨质疏松症(OP)是ILD常见的并发症之一,表现为骨质量和骨强度降低.中医方面,ILD、OP分别属于"肺痿""骨痿"范畴.ILD合并OP,二者相互影响及共存给临床诊治带来极大挑战,严重影响患者生存质量.TGF-β1/Smad信号传导通路在间质纤维化和骨代谢中呈现正负反馈调节.异常的TGF-β1/Smad信号传导与肺热密切相关,扰乱机体内环境稳态,在ILD合并OP的发生发展中扮演重要角色.本文基于中医学"诸痿发于肺热"理论,探讨间质性肺病并发骨质疏松症时TGF-β1/Smad信号传导通路的作用机制,以期为临床诊治提供新的理论依据和靶向治疗思路.

Interstitial lung disease(ILD)is a group of diseases with diffuse alveolitis and interstitial fibrosis as the main pathological manifestations.Osteoporosis(OP)is one of the common complications of ILD,which is mani-fested by decreased bone mass and bone strength.In Chinese medicine,ILD and OP belong to the categories of"lung impotence"and"bone impotence",respectively,and the interaction and coexistence of ILD and OP pose a great chal-lenge to clinical diagnosis and treatment,which seriously affects the quality of patient's survival.The TGF-β1/Smad signaling pathway shows positive and negative feedback regulation in interstitial fibrosis and bone metabolism.Ab-normal TGF-β1/Smad signaling is closely related to lung fever,which disturbs the homeostasis of the internal envi-ronment and plays an important role in the development of ILD combined with OP.Based on the theory of"all impo-tence originates from lung heat"in Chinese medicine,this article discusses the mechanism of TGF-β1/Smad signaling pathway in ILD with osteoporosis,with the aim of providing a new theoretical basis for clinical diagnosis and treat-ment and ideas for targeted therapy.
