目的:探究口腔扁平苔藓患者血清膜联蛋白A1(annexin A1,ANXA1)、C-X-C趋化因子受体 3(C-X-C chemokine receptor 3,CXCR3)表达及其与病损程度、辅助性T细胞 17(T helper cell 17,Th17)/调节性T细胞(regula-tory T cell,Treg)平衡的关系.方法:选取 2022 年 7 月~2023 年 7 月于本院确诊的 120 例口腔扁平苔藓患者为研究组,根据其临床分型分为非糜烂组(45 例)和糜烂组(75 例),糜烂组进一步根据糜烂程度分为轻中度糜烂组(44 例)和重度糜烂组(31 例);同时选取于本院体检的 120 例志愿者作为对照组.ELISA法检测血清ANXA1、CXCR3 水平;流式细胞术检测外周血 Th17、Treg比例;Spearman 或 Pearson 法分析血清 ANXA1、CXCR3 水平与疾病严重程度、Th17、Treg比例及Th17/Treg的相关性;多因素logistic回归分析影响口腔扁平苔藓患者糜烂程度的因素;受试者工作特征曲线(receiver operating characteristic,ROC)分析血清ANXA1、CXCR3 水平对口腔扁平苔藓患者糜烂程度的诊断价值.结果:与对照组相比,研究组血清ANXA1 水平显著降低,CXCR3 水平显著升高(P<0.05).与非糜烂组相比,糜烂组血清ANXA1 水平和Treg占比显著降低,CXCR3 水平、Th17 占比和Th17/Treg值显著升高(P<0.05);且重度糜烂组患者各指标变化程度较轻中度糜烂组更高(P<0.05).血清 ANXA1 水平与疾病严重程度、Th17 占比、Th17/Treg呈负相关,与Treg占比呈正相关(P<0.05);血清CXCR3 水平与疾病严重程度、Th17 占比、Th17/Treg呈正相关,与Treg占比呈负相关(P<0.05).ANXA1、Treg水平降低及CXCR3、Th17、Th17/Treg水平升高是影响口腔扁平苔藓患者重度糜烂的危险因素(P<0.05).血清ANXA1、CXCR3单独诊断口腔扁平苔藓患者重度糜烂的曲线下面积(the area under the curve,AUC)分别为 0.910、0.892,联合诊断AUC为 0.967,显著高于二者单独诊断(Z二者联合-ANXA1=2.161、Z二者联合-CXCR3=2.466,P<0.05).结论:口腔扁平苔藓患者血清ANXA1 水平降低,CXCR3 水平升高,与病损程度及Th17/Treg平衡密切相关,ANXA1 与CXCR3 联合检测对口腔扁平苔藓患者糜烂程度具有较高诊断价值.

Objective:To investigate the expression of serum annexin A1(ANXA1)and C-X-C chemokine receptor 3(CXCR3)in patients with oral lichen planus(OLP),and their relationship with the lesion severity and the T helper cell 17(Th17)/regulatory T cell(Treg)balance.Methods:From July 2022 to July 2023,120 patients diagnosed with OLP in our hospital were included as the study group.Patients were clinically classified into a non-erosive group(45 cases)and an ero-sive group(75 cases).The erosive group was further subdivided based on erosion severity into a mild-to-moderate erosion group(44 cases)and a severe erosion group(31 cases).Meantime,120 volunteers who experienced physical examinations at our hospital were designated as the control group.ELISA method was used to detect the levels of ANXA1 and CXCR3 in serum.Flow cytometry was used to detect the proportions of Th17 and Treg in peripheral blood.Spearman or Pearson methods were used to analyze the correlation between serum ANXA1,CXCR3 levels with disease severity,Th17,Treg ratios,and Th17/Treg.Multivariate logistic regression was used to analyze the factors affecting the severity of erosion in patients with OLP.Receiver operating characteristic(ROC)curves were used to analyze the diagnostic value of serum ANXA1 and CXCR3 levels for the severity of erosion in patients with OLP.Results:Compared with the control group,the study group had promi-nently lower serum ANXA1 and prominently higher CXCR3(P<0.05).Compared with the non erosion group,the erosion group had prominently lower serum ANXA1 level and Treg ratio,and prominently higher CXCR3 level,Th17 ratio,and Th17/Treg value(P<0.05),and the severe erosion group had a higher degree of change in various indicators than the mild to mod-erate erosion group(P<0.05).Serum ANXA1 was prominently negatively correlated with disease severity,Th17 proportion,and Th17/Treg,and prominently positively correlated with Treg proportion(P<0.05),serum CXCR3 was prominently posi-tively correlated with disease severity,Th17 proportion,and Th17/Treg,and prominently negatively correlated with Treg pro-portion(P<0.05).The decrease in ANXA1 and Treg levels,and the increase in CXCR3,Th17,and Th17/Treg levels were risk factors for severe erosion in patients with OLP(P<0.05).The area under the curve(AUC)values of serum ANXA1 and CXCR3 alone in the diagnosis of severe erosion in patients with OLP were 0.910 and 0.892,respectively.The AUC of com-bined diagnosis was 0.967,which was prominently higher than the individual diagnosis of the two(Zcombination-ANXA1=2.161,Zcombination-CXCR3=2.466,P<0.05).Conclusion:Serum ANXA1 is decreased,and CXCR3 is increased in patients with OLP,which is closely related to the severity of lesion and Th17/Treg balance.The combined detection of ANXA1 and CXCR3 has high diagnostic value for the severity of erosion in patients with OLP.
