中老年牙周炎患者龈沟液中Epstein-Barr病毒DNA载量的检测及其暴露因素研究OA
Detection of Epstein-Barr virus DNA load in gingival crevicular fluid and its associated exposure factors in middle-aged and elderly patients with periodontitis
目的:检测中老年牙周炎患者龈沟液(gingival crevicular fluid,GCF)中Epstein-Barr病毒(Epstein-Barr virus,EBV)DNA载量,分析主动吸烟、被动吸烟及饮酒等生活方式暴露因素对其分布水平的影响.方法:纳入 101例中老年牙周炎患者,通过问卷调查收集吸烟、被动吸烟及饮酒等流行病学资料;利用qPCR技术对GCF样本进行EBV DNA绝对定量分析.采用非参数检验比较不同暴露组间的载量差异,并构建多元线性回归模型评价各因素对病毒载量的独立影响.结果:101 例患者GCF中EBV DNA的中位载量为 677.16 IU/mL.单因素分析显示,目前吸烟组EBV载量显著高于不吸烟组(P<0.05,r=0.25);被动吸烟暴露组的病毒载量亦显著高于无暴露组(P<0.05,r=0.21).未发现饮酒习惯与EBV载量存在显著关联(P>0.05).多元线性回归显示,在校正年龄与性别后,烟草暴露仍与病毒载量升高呈现正向关联趋势.结论:主动及被动烟草暴露均与中老年牙周炎患者局部EBV载量升高显著相关.临床评估牙周病毒负荷时应充分考虑生活方式暴露因素的影响,烟草暴露可能是促进口腔EBV再激活的重要环境风险因素.
Objective:To detect the load of Epstein-Barr virus(EBV)DNA in the gingival crevicular fluid(GCF)of middle-aged and elderly patients with periodontitis and to analyze the impact of lifestyle exposure factors,including active smoking,passive smoking,and alcohol consumption,on its distribution.Methods:A total of 101 middle-aged and elderly pa-tients with periodontitis were enrolled.Epidemiological data regarding smoking,passive smoking,and drinking habits were col-lected via questionnaires.Absolute quantification of EBV DNA in GCF samples was performed using qPCR.Non-parametric tests were used to compare EBV loads across different exposure groups,and a multiple linear regression model was construc-ted to evaluate the independent effects of these factors on viral load.Results:The median EBV DNA load in GCF of the 101 patients was 677.16 IU/mL.Univariate analysis revealed that EBV loads in current smokers were significantly higher than those in non-smokers(P<0.05,r=0.25).Similarly,the passive smoking exposure group exhibited significantly higher viral loads than the non-exposed group(P<0.05,r=0.21).No significant association was found between drinking habits and EBV load(P>0.05).Multiple linear regression showed that after adjusting for age and sex,tobacco exposure remained positively associated with increased viral loads.Conclusion:Both active and passive tobacco exposure are significantly associated with elevated local EBV DNA loads in middle-aged and elderly patients with periodontitis.Tobacco exposure may serve as a criti-cal environmental risk factor for oral EBV reactivation,and lifestyle factors should be carefully considered when assessing per-iodontal viral status.
