目的 探讨急性脑梗死患者血清膜联蛋白A2(ANXA2)、谷胱甘肽过氧化物酶(GSH-Px)水平与梗死程度的关系及对介入术后预后不良的预测价值.方法 选择2022年8月至2024年8月于该院接受血管介入治疗的120例急性脑梗死患者(脑梗死组)及120例健康对照者(健康组)作为研究对象.根据梗死面积,将急性脑梗死患者分为重度梗死组、中度梗死组及轻度梗死组;根据患者出院3个月后的改良Rankin量表评分将患者分为预后不良组和预后良好组.采用酶联免疫吸附试验检测所有研究对象血清ANXA2水平;采用比色法检测所有研究对象血清GSH-Px水平;采用Spearman相关分析急性脑梗死患者血清ANXA2、GSH-Px水平与梗死程度的关系;采用多因素Logistic回归分析急性脑梗死患者介入术后预后不良的影响因素;采用受试者工作特征(ROC)曲线分析血清ANXA2、GSH-Px对急性脑梗死患者介入术后预后不良的预测效能.结果 脑梗死组血清ANXA2水平高于健康组(P<0.05),GSH-Px水平低于健康组(P<0.05).重度梗死组血清ANXA2水平高于中度梗死组和轻度梗死组(P<0.05),中度梗死组血清ANXA2水平高于轻度梗死组(P<0.05),重度梗死组血清GSH-Px水平低于中度梗死组和轻度梗死组(P<0.05),中度梗死组血清GSH-Px水平低于轻度梗死组(P<0.05).脑梗死组血清ANXA2水平与梗死程度呈正相关(P<0.05),GSH-Px水平与梗死程度呈负相关(P<0.05).预后不良组梗死面积、美国国立卫生研究院卒中量表评分及ANXA2水平高于预后良好组(P<0.05),GSH-Px水平低于预后良好组(P<0.05).梗死面积、ANXA2水平升高为急性脑梗死患者预后不良的危险因素(P<0.05),GSH-Px水平升高为保护因素(P<0.05).ANXA2、GSH-Px联合预测急性脑梗死患者预后不良的曲线下面积(AUC)为0.943,大于ANXA2(Z=2.176,P<0.05)、GSH-Px(Z=2.031,P<0.05)单独预测的AUC.结论 急性脑梗死患者血清ANXA2水平升高,GSH-Px水平降低,二者与脑梗死程度存在相关性,2项联合检测对介入术后预后不良具有较高的预测价值.

Objective To investigate the relationship of serum annexin A2(ANXA2)and glutathione per-oxidase(GSH-Px)levels with infarction severity and their predictive value for poor prognosis after interven-tional therapy in patients with acute cerebral infarction(ACI).Methods A total of 120 patients with ACI who underwent interventional therapy at the hospital from August 2022 to August 2024(the ACI group)and 120 healthy controls(the control group)were enrolled as research subjects.The ACI patients were categorized into severe,moderate and mild infarction groups based on infarct size,and were also divided into poor and good prognosis groups according to the modified Rankin Scale(mRS)score at 3 months post-surgery.Serum ANXA2 level of all research subjects was measured by enzyme-linked immunosorbent assay,and GSH-Px lev-el of all research subjects was determined by colorimetry.Spearman correlation analysis was performed to e-valuate the relationship of ANXA2 and GSH-Px levels with infarction severity in ACI patients.Multivariate Logistic regression analysis was applied to identify factors affecting the poor prognosis of ACI patients after interventional therapy.Receiver operating characteristic(ROC)curve was used to assess the predictive value of serum ANXA2 and GSH-Px levels for poor prognosis of ACI patients after interventional therapy.Results Serum ANXA2 level in the ACI group was found to be significantly higher than that in the control group(P<0.05),whereas GSH-Px level was significantly lower(P<0.05).Serum ANXA2 level in the se-vere infarction group was higher than that in the moderate and mild infarction groups(P<0.05),and serum ANXA2 level in the moderate group was higher than that in the mild group(P<0.05).Conversely,GSH-Px level in the severe infarction group was lower than that in the moderate and mild infarction groups(P<0.05),with level in the moderate group being lower than that in the mild group(P<0.05).Serum ANXA2 level was positively correlated with infarction severity(P<0.05),and GSH-Px level was negatively correlated with infarction severity(P<0.05).Infarct size,National Institutes of Health Stroke Scale score and ANXA2 level in the poor prognosis group were higher than those in the good prognosis group(P<0.05),while GSH-Px level was lower(P<0.05).Larger infarct size and elevated ANXA2 level were identified as independent risk factors for poor prognosis(P<0.05),while elevated GSH-Px level was determined to be a protective fac-tor(P<0.05).The area under the curve(AUC)for the combination of ANXA2 and GSH-Px in predicting poor prognosis was calculated as 0.943,which was significantly greater than the AUC for ANXA2 alone(Z=2.176,P<0.05)and GSH-Px alone(Z=2.031,P<0.05).Conclusion Serum ANXA2 level is elevated and GSH-Px level is decreased in patients with ACI.Both biomarkers show significant correlations with the severi-ty of cerebral infarction.Their combined detection demonstrates high predictive value for poor prognosis after interventional therapy.
