急性缺血性脑卒中血清Ang-1、ANXA2水平变化及与预后的关系OA
Changes in serum Ang-1 and ANXA2 levels and their association with prognosis in acute ischemic stroke
目的 探讨急性缺血性脑卒中(AIS)患者血清血管生成素-1(Ang-1)、膜联蛋白A2(ANXA2)水平变化及与预后的关系.方法 选取2021年10月至2023年8月该院收治的120例AIS患者作为研究对象,根据出院后3个月预后情况,将其分为预后不良组、预后良好组.比较2组基线资料,治疗前、治疗后血清Ang-1、ANXA2水平及ΔAng-1、ΔANXA2,分析ΔAng-1、ΔANXA2与预后不良风险的剂量-反应关系.将ΔAng-1、ΔANXA2分为4个亚组:Q1亚组(高ΔAng-1高ΔANXA2)、Q2亚组(低ΔAng-1高ΔANXA2)、Q3亚组(高ΔAng-1低ΔANXA2)、Q4亚组(低ΔAng-1低ΔANXA2),比较4亚组预后不良率,分析ΔAng-1、ΔANXA2对AIS患者预后不良的影响,并评估其对AIS患者预后不良的预测价值.结果 120例AIS患者近期预后不良率为30.83%.预后不良组发病至治疗时间、治疗前美国国立卫生研究院卒中量表评分及血清B型钠尿肽、C反应蛋白水平均高于预后良好组(P<0.05).2组治疗后血清Ang-1水平均高于治疗前(P<0.05),ANXA2水平均低于治疗前(P<0.05);预后不良组治疗前、治疗后血清Ang-1水平及ΔAng-1、ΔANXA2均低于预后良好组(P<0.05),治疗前、治疗后血清ANXA2水平均高于预后良好组(P<0.05).ΔAng-1、ΔANXA2与AIS患者预后不良风险存在线性剂量-反应关系(P<0.05).4个亚组预后不良率比较,差异有统计学意义(P<0.05).ΔAng-1<0.36 ng/mL、ΔANXA2<12.31 μg/L均为AIS患者预后不良的独立危险因素(P<0.05).ΔAng-1、ΔANXA2单独预测AIS患者预后不良的曲线下面积(AUC)分别为0.753、0.760,ΔAng-1联合ΔANXA2预测的AUC为0.877,明显大于ΔAng-1、ΔANXA2单独预测的AUC(P<0.05).结论 血清ΔAng-1、ΔANXA2与AIS患者预后不良风险存在线性剂量-反应关系,ΔAng-1与ΔANXA2联合检测显示出良好的预测预后潜力,可能为临床风险评估提供参考.
Objective To investigate the changes in serum angiopoietin-1(Ang-1)and annexin A2(ANXA2)levels and their association with prognosis in patients with acute ischemic stroke(AIS).Methods A total of 120 patients with AIS admitted to this hospital from October 2021 to August 2023 were enrolled as research subjects.Based on their 3-month post-discharge prognosis,the AIS patients were assigned to a poor-prognosis group or a good-prognosis group.Baseline characteristics were compared between the two groups.Levels of serum Ang-1 and ANXA2 before and after treatment,along with their absolute changes(ΔAng-1 and ΔANXA2),were compared.The dose-response relationships of ΔAng-1 and ΔANXA2 with poor prognosis risk were analyzed.Patients were further categorized into four subgroups based on ΔAng-1 and ΔANXA2:Q1 subgroup(high ΔAng-1/high ΔANXA2),Q2 subgroup(low ΔAng-1/high ΔANXA2),Q3 sub-group(high ΔAng-1/low ΔANXA2)and Q4 subgroup(low ΔAng-1/low ΔANXA2).The incidence of poor prognosis was compared across subgroups to evaluate the effects and predictive value of ΔAng-1 and ΔANXA2 for poor prognosis of AIS patients.Results The incidence of poor short-term prognosis among the 120 pa-tients with AIS was 30.83%.The time from onset to treatment,the pre-treatment National Institutes of Health Stroke Scale score,and the serum levels of B-type natriuretic peptide and C-reactive protein were sig-nificantly higher in the poor-prognosis group than those in the good-prognosis group(P<0.05).Serum Ang-1 levels were significantly increased after treatment in both groups(P<0.05),whereas serum ANXA2 levels were significantly decreased after treatment in both groups(P<0.05).Serum Ang-1 levels before and after treatment,as well as ΔAng-1 and ΔANXA2,were significantly lower in the poor-prognosis group than those in the good-prognosis group(P<0.05),while serum ANXA2 levels before and after treatment were significant-ly higher in the poor-prognosis group(P<0.05).Linear dose-response relationships were observed between ΔAng-1,ΔANXA2 and poor prognosis risk in AIS patients(P<0.05).There was a statistically significant difference in the incidence of poor prognosis among the four subgroups(P<0.05).ΔAng-1<0.36 ng/mL and ΔANXA2<12.31 μg/L were identified as independent risk factors for poor prognosis in patients with AIS(P<0.05).The areas under the curve(AUCs)for predicting poor prognosis in AIS patients was 0.753 for ΔAng-1 alone and 0.760 for ΔANXA2 alone,whereas the combined prediction of ΔAng-1 and ΔANXA2 yiel-ded an AUC of 0.877,which was significantly greater than that of either marker alone(P<0.05).Conclusion A linear dose-response relationship exists between ΔAng-1,ΔANXA2 and poor prognosis risk in AIS patients.The combined detection of ΔAng-1 and ΔANXA2 shows promising potential for prognostic pre-diction,which may provide a reference for clinical risk assessment.
