目的 探讨血清成纤维细胞生长因子23(FGF-23)、C1q/肿瘤坏死因子相关蛋白3(CTRP3)和微小RNA-30b(miR-30b)对维持性血液透析(MHD)患者血管钙化的预测价值.方法 选取该院2022年1月至2024年12月收治的MHD患者220例作为研究对象,采用X线片检查血管钙化情况,依据Kauppila钙化积分分为无钙化组(0分)、轻度钙化组(1~4分)和中重度钙化组(>4分).比较3组基线资料,以及血清FGF-23、CTRP3和miR-30b水平,分析MHD患者血清学指标与血管钙化程度的相关性.采用受试者工作特征(ROC)曲线分析血清FGF-23、CTRP3和miR-30b对MHD患者血管钙化的预测价值.结果 中重度钙化组透析时长长于轻度钙化组、无钙化组(P<0.05),轻度钙化组透析时长长于无钙化组(P<0.05).中重度钙化组血钙(Ca)、血磷(P)、甲状旁腺激素(PTH)、FGF-23和CTRP3水平高于轻度钙化组和无钙化组(P<0.05),miR-30b水平低于轻度钙化组和无钙化组(P<0.05).轻度钙化组Ca、P、PTH、FGF-23和CTRP3水平高于无钙化组(P<0.05),miR-30b水平低于无钙化组(P<0.05).相关性分析结果显示,MHD患者透析时长、血清Ca、P、PTH、FGF-23和CTRP3水平与血管钙化程度呈正相关(P<0.05),miR-30b水平与血管钙化程度呈负相关(P<0.05).ROC曲线分析结果显示,FGF-23、CTRP3和miR-30b的预测 MHD患者血管钙化的曲线下面积分别为0.894、0.901和0.844(P<0.05).结论 MHD发生血管钙化患者血清FGF-23和CTRP3水平升高,miR-30b水平降低,FGF-23、CTRP3、miR-30b均与MHD发生血管钙化有关,可作为临床预测的参考指标.

Objective To investigate the predictive value of serum fibroblast growth factor 23(FGF-23),C1q/tumor necrosis factor-related protein 3(CTRP3)and microRNA-30b(miR-30b)for vascular calcification(VC)in maintenance hemodialysis(MHD)patients.Methods A total of 220 MHD patients treated at the hospital from January 2022 to December 2024 were selected as study subjects.VC status was assessed using X-ray radiography,and patients were categorized into three groups based on the Kauppila calcification score:a non-calcification group(0 points),a mild calcification group(1-4 points)and a moderate-to-severe calcifica-tion group(>4 points).Baseline characteristics,as well as serum levels of FGF-23,CTRP3 and miR-30b,were compared among the three groups.The correlation between these serum biomarkers and the degree of VC was analyzed.The predictive value of serum FGF-23,CTRP3 and miR-30b for VC in MHD patients was evaluated using receiver operating characteristic(ROC)curve analysis.Results Dialysis duration was significantly lon-ger in the moderate-to-severe calcification group compared with the mild and non-calcification groups(P<0.05),and it was also significantly longer in the mild calcification group than that in the non-calcification group(P<0.05).Serum levels of calcium(Ca),phosphorus(P),parathyroid hormone(PTH),FGF-23 and CTRP3 were significantly higher in the moderate-to-severe group than those in the other two groups(P<0.05),while miR-30b level was lower(P<0.05).Similarly,the levels of Ca,P,PTH,FGF-23 and CTRP3 in the mild calcification group were significantly higher than those in the non-calcification group(P<0.05),whereas the miR-30b level was lower(P<0.05).Dialysis duration and serum levels of Ca,P,PTH,FGF-23 and CTRP3 were positively correlated with the degree of VC(P<0.05),whereas miR-30b level was negative-ly correlated(P<0.05).ROC curve analysis showed that the areas under the curve of FGF-23,CTRP3 and miR-30b for predicting VC were 0.894,0.901 and 0.844 respectively(P<0.05).Conclusion In MHD pa-tients with VC,serum levels of FGF-23 and CTRP3 are elevated,while miR-30b level is decreased.FGF-23,CTRP3 and miR-30b are all associated with the development of VC in MHD patients and can serve as predic-tive biomarkers in clinical practice.
