清瘟败毒饮在热毒蕴结型上尿路结石梗阻致尿源性脓毒血症患者术后的应用效果OA
Effect of Qingwen Baidu Decoction in postoperative management of urosepsis patients with up per urinary tract stone obstruction and heat-toxin accumulation syndrome
目的 探讨清瘟败毒饮在热毒蕴结型上尿路结石梗阻致尿源性脓毒血症患者术后的应用效果.方法 选取2024 年 7 月至 2025 年 5 月福建中医药大学附属晋江中医院泌尿外科收治的 60 例热毒蕴结型上尿路结石梗阻致尿源性脓毒血症患者作为研究对象,按照随机数字表法分为治疗组(30 例)与对照组(30 例),对照组术后予抗生素抗感染、对症治疗、监测生命征,治疗组在对照组基础上服用清瘟败毒饮,比较两组中医证候积分、急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)、白细胞计数(WBC)、降钙素原(PCT)、C反应蛋白(CRP)水平及疗效评定.结果 两组患者治疗前的中医证候积分、APACHEⅡ、WBC、CRP、PCT比较,差异无统计学意义(P＞0.05).治疗后,治疗组中医证候积分、APACHEⅡ评分、WBC、CRP、PCT水平均低于对照组,差异有统计学意义(P＜0.05).结论 清瘟败毒饮在热毒蕴结型上尿路结石梗阻致尿源性脓毒血症患者术后可明显改善症状,抑制炎症反应,缩短病程,促进康复.
Objective To explore the effect of Qingwen Baidu Decoction in postoperative management of urosepsis patients with upper urinary tract stone obstruction and heat-toxin accumulation syndrome.Methods A total of 60 patients with urosepsis caused by heat-toxin accumulation syndrome upper urinary tract stone obstruction who met the criteria and were admitted to the Department of Urology,Jinjiang Hospital of Traditional Chinese Medicine,Fujian University of Traditional Chinese Medicine from July 2024 to May 2025 were selected as the study subjects,and they were divided into the treatment group(30 cases)and the control group(30 cases)by random number table method.The control group was given antibiotics to fight infection,symptomatic treatment,and vital signs were monitored after surgery,while the treatment group took Qingwen Baidu Decoction on the basis of the control group.The traditional Chinese medicine syndrome scores,acute physiology and chronic health evaluationⅡ(APACHE Ⅱ)scores,white blood cell(WBC)counts,procalcitonin(PCT)and C-reactive pro-tein(CRP)levels,as well as therapeutic efficacy were compared between the two groups.Results There were no statistically significant differences in traditional Chinese medicine syndrome scores,APACHEⅡscores,WBC,CRP,or PCT levels be-tween the two patient groups before treatment(P＞0.05).After treatment,the traditional Chinese medicine syndrome score,APACHEⅡscore,and the levels of WBC,CRP,and PCT in the treatment group were lower than those in the control group,and the differences were statistically significant(P＜0.05).Conclusion Qingwen Baidu Decoction can significantly improve symptoms,inhibit inflammatory response,shorten the course of the disease,and promote recovery in patients with urosepsis caused by upper urinary tract stone obstruction caused by heat-toxin accumulation type.
