中药烫熨联合香砂六君子汤在重症监护室机械通气腹胀患者中的应用效果OA
Application effect of traditional Chinese medicine ironing combined with Xiangsha Liujunzi Decoction in patients with abdominal distension undergoing mechanical ventilation in the intensive care unit
目的 探讨中药烫熨联合香砂六君子汤在重症监护室(ICU)机械通气腹胀患者中的应用效果.方法 选取2022 年 7 月至 2023 年 12 月萍乡市中医院ICU收治的 80 例机械通气腹胀患者作为研究对象,按照随机数字表法分为对照组和观察组,每组 40 例.对照组予以中药烫熨,观察组在对照组的基础给予香砂六君子汤治疗.比较两组治疗前后中医证候积分、临床指标、腹内压(IAP)、动脉血气、临床疗效及不良反应.结果 治疗后,观察组中医证候积分低于对照组,差异有统计学意义(P＜0.05);观察组机械通气时间、ICU住院时间、肠鸣音恢复时间、首次排气时间均短于对照组,腹围缩小值大于对照组,差异有统计学意义(P＜0.05);治疗后,观察组IAP水平低于对照组,pH、动脉血氧分压(PaO2)水平均高于对照组,动脉血二氧化碳分压(PaCO2)低于对照组,差异有统计学意义(P＜0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P＜0.05);两组患者不良反应发生率比较,差异无统计学意义(P＞0.05).结论 中药烫熨联合香砂六君子汤可降低ICU机械通气腹胀患者IAP,促进胃肠功能恢复,有效缓解腹胀,改善动脉血气,临床疗效好,且安全性较高,值得在ICU腹胀管理中推广.
Objective To explore the application effect of traditional Chinese medicine ironing combined with Xiangsha Liujunzi Decoction in patients with abdominal distension undergoing mechanical ventilation in the intensive care unit(ICU).Methods A total of 80 patients experiencing mechanically ventilated abdominal distension who were admitted to the ICU of Pingxiang Traditional Chinese Medicine Hospital from July 2022 to December 2023 were selected as the study subjects,and they were divided into the control group and the observation group by random number table method,with 40 patients in each.The control group received traditional Chinese medicine ironing treatment,while the observation group was administered Xiangsha Liujunzi Decoction in addition to the control group's treatment.The traditional Chinese medicine syndrome scores,clinical indicators,intra-abdominal pressure(IAP),arterial blood gas levels,clinical efficacy,and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the traditional Chinese medicine syndrome score of the observation group were lower than those of the control group,and the differences were statistically significant(P＜0.05).The duration of mechanical ventilation,ICU stay time,bowel sounds recovery time,and first exhaust time of the observation group were shorter than those of the control group,while the reduction in abdominal circumference was greater than that in the control group,and the differences were statistically significant(P＜0.05).After treatment,the IAP in the observation group was lower than that in the control group,whereas the levels of pH and arterial partial pres-sure of oxygen(PaO2)were higher than those in the control group,and the arterial partial pressure of carbon dioxide(PaCO2)was lower than that in the control group,and the differences were statistically significant(P＜0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P＜0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P＞0.05).Conclusion The combination of traditional Chinese medicine ironing and Xiangsha Liujunzi Decoction can reduce IAP in ICU mechanically ventilated patients with abdominal distension,promote gastrointestinal function recovery,effectively alleviate abdominal distension,improve arterial blood gas,and have good clinical efficacy and high safety.It is worth promoting in ICU abdominal distension management.
