玻璃体视网膜手术联合地塞米松玻璃体内植入剂治疗增殖型糖尿病性视网膜病变的效果OA
Effect of vitreoretinal surgery combined with Dexamethasone intravitreal implant in the treatment of proliferative diabetic retinopathy
目的 探讨玻璃体视网膜手术联合地塞米松玻璃体内植入剂治疗增殖型糖尿病性视网膜病变(PDR)的效果.方法 选取 2022 年 3 月至 2025 年 3 月上饶爱尔眼科医院收治的 89 例PDR患者作为研究对象,根据治疗方案分为研究组(n=45)、对照组(n=44).对照组采用玻璃体视网膜手术,研究组在对照组基础上采用地塞米松玻璃体内植入剂.比较两组的治疗效果、视力状况、生活质量、并发症.结果 研究组的总有效率高于对照组,差异有统计学意义(P＜0.05).两组患者治疗后的黄斑中心凹厚度(CMT)、眼压小于本组治疗前,最佳矫正视力(BCVA)高于本组治疗前,且研究组的CMT、眼压小于对照组,BCVA高于对照组,差异有统计学意义(P＜0.05).两组患者治疗后的生理职能、生理功能、一般健康、心理健康分值高于本组治疗前,且研究组评分高于对照组,差异有统计学意义(P＜0.05).研究组的并发症总发生率低于对照组,差异有统计学意义(P＜0.05).结论 玻璃体视网膜手术联合地塞米松玻璃体内植入剂治疗PDR,可有效改善视功能,提升整体疗效,并能降低并发症发生风险,对提高患者生活质量具有积极意义.
Objective To explore the efficacy of vitreoretinal surgery combined with intravitreal implantation of Dexametha-sone in the treatment of proliferative diabetic retinopathy(PDR).Methods A total of 89 patients with PDR admitted to Shangrao Aier Eye Hospital from March 2022 to March 2025 were selected as the research subjects,they were divided into the study group(n=45)and the control group(n=44)according to the treatment plan.The control group underwent vitreoreti-nal surgery,while the study group received Dexamethasone intravitreal implant in addition to the treatment of the control group.The therapeutic effects,visual acuity,quality of life,and complications of the two groups were compared.Results The overall effective rate of the research group was higher than that of the control group,and the difference was statistically sig-nificant(P＜0.05).After treatment,the central macular thickness(CMT),eye pressure of the patients in both groups was smaller than that before treatment,the best corrected visual acuity(BCVA)was higher than that before treatment,and the CMT and eye pressure in the research group were smaller than those in the control group,while the BCVA was higher than that in the control group,the differences were statistically significant(P＜0.05).The scores of physical function,physical a-bility,general health,and mental health of the patients in both groups after treatment were higher than those before treatment,and scores of the research group were higher than those of the control group,with statistically significant differences(P＜0.05).The total incidence of complications in the research group was lower than that in the control group,and the difference was statistically significant(P＜0.05).Conclusion The combined treatment of vitreoretinal surgery and intravitreal implantation of Dexamethasone for PDR can effectively improve visual function,enhance overall therapeutic effect,and reduce the risk of complications.This approach holds significant positive implications for improving the quality of life of patients.
