补火汤佐治慢性心力衰竭心肾阳虚证疗效观察OA
Observation on the Efficacy of Buhuo Decoction in Treating Chronic Heart Failure with Heart-kidney Yang Deficiency Syndrome
目的 观察补火汤对慢性心力衰竭(CHF)心肾阳虚证患者的临床疗效.方法 选取2022年12月—2025年4月南阳市中心医院收治的CHF心肾阳虚证患者98例作为研究对象,按信封法分为对照组和观察组各49例.对照组给予卡托普利、螺内酯、琥珀酸美托洛尔、达格列净等常规西药口服治疗,观察组在此基础上给予补火汤治疗.经治3个月,比较两组患者中医证候(主症、次症、总分)评分、临床疗效、炎症指标[可溶性CD40配体(sCD40L)、白介素-1β(IL-1β)、白介素-6(IL-6)]水平、心脏指标[左室射血分数(LVEF)、心脏指数(CI)]水平,以及不良反应(心悸、头晕、胃肠损伤、肝肾功能异常等)发生情况.结果 治疗后,观察组中医证候评分低于对照组(P＜0.05);观察组临床总有效率为93.88％(46/49),优于对照组的79.59％(39/49)(P＜0.05);观察组sCD40L、IL-1β、IL-6水平均低于对照组(P＜0.05);LVEF、CI均高于对照组(P＜0.05);观察组不良反应总发生率为8.16％(4/49),对照组为4.08％(2/49),组间差异无统计学意义(P＞0.05).结论 补火汤佐治CHF心肾阳虚证,可改善临床症状和心功能,降低机体炎症因子水平,提高临床疗效,且安全性良好.
Objective To observe the clinical efficacy of Buhuo Decoction in patients with chronic heart failure(CHF)of heart-kidney yang deficiency syndrome(HKYDS).Methods A total of 98 patients with CHF of heart-kidney yang deficiency syndrome admitted to Nanyang Central Hospital from December 2022 to April 2025 were selected as the study subjects and divided into a control group and an observation group,with 49 cases in each group,using the envelope method.The control group was treated with conventional western medicine oral-ly,including captopril,spironolactone,metoprolol,and dapagliflozin.The observation group received additional treatment with Buhuo Decoction on this basis.After 3 months of treatment,the two groups were compared regard-ing symptom scores(primary symptoms,secondary symptoms,and total scores),clinical efficacy,levels of in-flammatory markers[soluble CD40 ligand(sCD40L),interleukin-1 β(IL-1β),and interleukin-6(IL-6)],cardiac markers[left ventricular ejection fraction(LVEF)and cardiac index(CI)],and the occur-rence of adverse reactions(palpitations,dizziness,gastrointestinal injury,abnormal liver and kidney function,etc).Results After treatment,the TCM symptom scores in the observation group were lower than those in the control group(P＜0.05);the overall clinical efficacy rate in the observation group was 93.88％(46/49),which was superior to that in the control group[79.59％(39/49)](P＜0.05);the levels of sCD40L,IL-1 β,and IL-6 in the observation group were lower than those in the control group(P＜0.05);the LVEF and CI in the observation group were higher than those in the control group(P＜0.05);the overall incidence of ad-verse reactions in the observation group was 8.16％(4/49),compared to 4.08％(2/49)in the control group,with no statistically significant difference between the groups(P＞0.05).Conclusion Buhuo Decoction as an adjunctive therapy for CHF of heart-kidney yang deficiency syndrome can improve clinical symptoms and cardi-ac function,reduce levels of inflammatory factors,and enhance clinical efficacy,with good safety.
