多囊卵巢综合征患者血清SAA1、PTX3表达与胰岛素抵抗的相关性OA
Correlation between serum SAA11,PTX3 levels and insulin resistance in patients with polycystic ovary syndrome
目的 探究多囊卵巢综合征(PCOS)患者血清淀粉样蛋白A1(SAA1)、正五聚蛋白3(PTX3)表达与胰岛素抵抗(IR)的相关性.方法 选取2022年5月至2024年4月于乐山市妇幼保健院进行治疗的PCOS患者179例作为研究组,根据稳态模型胰岛素抵抗指数(HOMA-IR)分为合并IR组83例和非合并IR组96例,选择同期健康体检人员95例作为对照组.使用酶联免疫吸附法(ELISA)法检测血清SAA1和PTX3水平,同时检测空腹血糖(FBG)、空腹胰岛素(FINS)等临床指标.使用Pearson相关分析合并IR组血清SAA1、PTX3水平与IR相关指标的相关性,使用Logistic回归分析PCOS患者IR发生的影响因素,使用受试者工作特征(ROC)曲线分析模型预测价值.结果 与对照组相比,研究组SAA1和PTX3表达水平上调,差异有统计学意义(t值分别为12.696、11.541,P<0.05);合并IR组的体质量指数(BMI)、FBG、FINS、HOMA-IR、SAA1和PTX3高于非合并IR组,差异有统计学意义(t值介于2.758~17.212之间,P<0.05);合并IR组血清SAA1和PTX3表达水平与BMI、FBG、FINS和HOMA-IR指标均呈正相关性(r值介于0.412~0.532之间,P<0.05);多因素Logistic回归分析结果显示SAA1和PTX3为PCOS患者IR发生的危险因素(OR值分别为2.189、1.547,P<0.05);血清SAA1和PTX3水平分别预测PCOS合并IR发生的曲线下面积(AUC)为0.788和0.793,二者联合预测的AUC为0.880,明显高于SAA1和PTX3单独预测(Z值分别为2.110、2.071,P<0.05).结论 多囊卵巢综合征患者血清SAA1和PTX3水平显著上调,多囊卵巢综合征合并胰岛素抵抗患者血清SAA1和PTX3水平显著高于非合并胰岛素抵抗组,血清SAA1和PTX3水平的检测有可能用于多囊卵巢综合征及其合并胰岛素抵抗病情的评估.
Objective To investigate the correlation between serum amyloid A1(SAA1)and pentraxin-3(PTX3)expression and insulin resistance(IR)in patients with polycystic ovary syndrome(PCOS).Methods From May 2022 to April 2024,179 PCOS patients who received treatment in Leshan Maternal and Child Health Hospital were included as the study group.According to the homeostasis model assessment of insulin resistance(HOMA-IR),they were grouped into a combined IR group of 83 cases and a non-IR group of 96 cases.Meanwhile,95 healthy individuals who underwent physical examinations were selected as the control group.Enzyme-linked immunosorbent assay(ELISA)method was applied to detect serum levels of SAA1 and PTX3.The clinical factors such as fasting blood glucose(FBG)and fasting insulin(FINS)were detected.Pearson correlation was applied to analyze the correlation between serum SAA1 and PTX3 levels and IR-related indicators in the combined IR group.Logistic regression was applied to analyze the influencing factors of IR in PCOS patients.The receiver operating characteristic(ROC)curve was applied to analyze the predictive value of the model.Results Compared with the control group,the expression levels of SAA1 and PTX3 in the study group were increased,and the difference was statistically significant(t=12.696,11.541,P<0.05);the BMI,FBG,FINS,HOMA-IR,SAA1 and PTX3 in the combined IR group were higher than those in the non-IR group,and the difference was statistically significant(t values ranged from 2.758 to 17.212,P<0.05);the expression levels of serum SAA1 and PTX3 in the combined IR group were positively correlated with the BMI,FBG,FINS and HOMA-IR indicators(r values ranged from 0.412 to 0.532,P<0.05).Multivariate logistic regression analysis showed that SAA1,and PTX3 were risk factors for IR occurrence in PCOS patients(OR=2.189,1.547,P<0.05).The area under the curve(AUC)for predicting IR occurrence in PCOS patients by serum SAA1 and PTX3 levels was 0.788 and 0.793 respectively.The AUC for the combined prediction was 0.880,which was significantly higher than that of the individual predictions of SAA1 and PTX3(Z=2.110,2.071,P<0.05).Conclusion The expression levels of serum SAA1 and PTX3 in PCOS patients are significantly elevated.The expression levels of serum SAA1 and PTX3 in PCOS patients combined with IR are significantly higher than those in the non-IR group.The detection of serum SAA1 and PTX3 levels may be used for evaluating disease status of PCOS patients and PCOS patients with IR.
