目的 探讨miR-423-5p和miR-328-3p在妊娠期糖尿病(GDM)患者中的表达及其与胰岛素抵抗的关系.方法 选取2022年4月至2024年4月期间在徐州市中心医院妇产科产检并接受GDM筛查的240例孕妇为研究对象,分为GDM(研究组,n=120)和正常妊娠(对照组,n=120).采用荧光定量PCR法检测 miR-423-5p和 miR-328-3p表达水平,对两组一般临床资料及胰岛素抵抗相关指标进行比较,Pearson相关法分析GDM患者miR-423-5p与miR-328-3p水平间及二者与胰岛素抵抗指标间的相关性;多因素Logistic回归分析影响GDM发生的因素.结果 研究组miR-423-5p、miR-328-3p水平均高于对照组,差异有统计学意义(t值分别为7.267、7.556,P<0.05);研究组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、空腹胰岛素(FINS)和胰岛素抵抗指数(HOMA-IR)高于对照组,高密度脂蛋白胆固醇(HDL-C)和胰岛β细胞功能指数(HOMA-β)低于对照组,差异有统计学意义(t值介于4.579~19.854之间,P<0.05);miR-423-5p与miR-328-3p水平之间呈正相关性(r=0.623,P<0.05);miR-423-5p和miR-328-3p与胰岛素抵抗指标TC、TG、LDL-C、FPG、FINS、HOMA-IR均呈正相关性(r值介于0.403~0.592之间,P<0.05),与 HDL-C和 HOMA-β均呈负相关性(r值介于-0.523~-0.368之间,P<0.05);多因素Logistic回归分析结果显示,miR-423-5p、miR-328-3p、TC、TG、LDL-C、FPG和FINS均是影响GDM发生的危险因素(OR值介于2.120~4.851之间,P<0.05).结论 妊娠期糖尿病患者miR-423-5p和miR-328-3p水平均异常升高,二者与胰岛素抵抗相关.

Objective To investigate the expression levels of miR-423-5p and miR-328-3p in pregnant women with gestational diabetes mellitus(GDM)and their correlation with insulin resistance.Methods From April 2022 to April 2024,240 pregnant women who underwent prenatal examination and GDM screening in the Department of Obstetrics and Gynecology of Xuzhou Central Hospital were enrolled in this study.They were divided into the GDM group(study group,n=120)and the normal pregnancy group(control group,n=120).The expression levels of miR-423-5p and miR-328-3p were detected by quantitative real-time PCR(qRT-PCR).The general clinical data and insulin resistance-related indicators were compared between the two groups.Pearson correlation analysis was used to analyze the correlation between the levels of miR-423-5p and miR-328-3p in GDM pregnant women,as well as the correlations between these two microRNAs and insulin resistance indicators.Multivariate logistic regression analysis was applied to identify the factors influencing the occurrence of GDM.Results The levels of miR-423-5p and miR-328-3p in the study group were significantly higher than those in the control group,with statistically significant differences(t-values=7.267 and 7.556,respectively,P<0.05).In terms of biochemical indicators,the study group had higher levels of TC,TG,LDL-C,FPG,FINS,and HOMA-IR,while lower levels of HDL-C and HOMA-β compared with the control group;all differences were statistically significant(t-values ranged from 4.579 to 19.854,P<0.05).Correlation analysis showed a positive correlation between the levels of miR-423-5p and miR-328-3p(r=0.623,P<0.05).Both miR-423-5p and miR-328-3p were positively correlated with the insulin resistance-related indicators(TC,TG,LDL-C,FPG,FINS,HOMA-IR)(r-values ranged from 0.403 to 0.592,P<0.05),and negatively correlated with HDL-C and HOMA-β(r-values ranged from-0.523 to-0.368,P<0.05).Results of multivariate logistic regression analysis indicated that miR-423-5p,miR-328-3p,TC,TG,LDL-C,FPG,and FINS were all risk factors for GDM(OR values ranged from 2.120 to 4.851,P<0.05).Conclusion The levels of miR-423-5p and miR-328-3p are abnormally elevated in patients with GDM,and both are associated with insulin resistance.
