基于"骨筋脉"理论探讨腰椎间盘突出症的推拿诊疗体系构建OA

The sinew-bone theory and meridian theory are the fundamental theories of traditional Chinese medicine(TCM)Tuina therapy for treating sinew injury disorders.Lumbar disc herniation(LDH)is the most common clinical disease with low back pain and leg pain as the main complaints.Upon diagnosis and examination,bone misalignment,sinew injury at the herniated disc segment,as well as meridian obstruction along the Bladder Meridian and Gallbladder Meridian are often found.Therefore,reduction,sinew manipulation and meridian dredging are regarded as the main Tuina treatment methods for LDH.With the influence of modern rehabilitation theory and the development of Tuina practice,Tuina diagnosis and treatment guided by the"bone-sinew-meridian"theory have gained richer connotations.On the basis of reviewing the transformation of the diagnostic and therapeutic thinking model for LDH from the perspective of sinew and bone,this article elaborates on the new connotation of the"bone-sinew-meridian"theory from the perspectives of sinew-bone theory,meridian theory and meridian-sinew theory,and constructs a"sinew-bone-based meridian and meridian-sinew theory",so as to form a three-dimensional Tuina diagnosis and treatment framework for LDH combining bone adjustment,sinew manipulation and meridian dredging.
