Painful heel syndrome(PHS)is a common foot and ankle disorder,whose etiology is mostly related to plantar fasciitis and calcaneal spurs.In traditional Chinese medicine(TCM),it is classified as a"Jingjin(sinew)disease",with the core pathogenesis of"sinew displacement from the groove and bone misalignment".Adhering to the Jingjin theory,Professor Wang Haidong founded the"Three-Ring"theory,holding that the human body is composed of three major regions:the"upper ring","middle ring"and"lower ring",which are connected and interpenetrated by the Twelve Jingjin to form an integral structure with dynamic balance.Guided by the principle of"simultaneous treatment of tendons and collaterals,and equal emphasis on the whole body and local area",he proposed the needle-knife treatment strategies of"taking pain points as acupoints and local release","treating based on syndrome differentiation along tendons and overall regulation",and"guiding tendon qi through vessels and balancing yin and yang",which mainly include local release technique,Quepen(Supraclavicular Fossa)knot-loosening technique and Chongmai(Thoroughfare Vessel)knot-loosening technique.This strategy aims to harmonize tendons and collaterals,unblock collateral branches,and reconstruct the overall tension distribution of Jingjin,thereby significantly relieving symptoms and restoring function.By presenting a representative case,this paper demonstrates the remarkable clinical efficacy of needle-knife therapy in treating PHS,providing a new clinical pathway and theoretical support for the treatment of PHS.
