目的:探讨说话瓣膜干预对脑卒中后气管切开患者呼吸肌功能及咳嗽效能的影响及安全性.方法:选取本院2022年6月~2025年2月住院的脑卒中气管切开患者100例,采用随机数字表法将其分为试验组和对照组各50例.对照组实施常规吞咽、呼吸康复训练,试验组在此基础上增加佩戴说话瓣膜训练,检测两组患者治疗前后的咳嗽峰值流量(CPF)和最大吸气压力(MIP)的变化.结果:经过3周治疗,试验组与对照组的最大吸气压力、咳嗽峰值流量较治疗前均有显著提升(R<0.05),试验组MIP及CPF的改善幅度显著大于对照组(交互效应P<0.05).结论:在实施常规吞咽、呼吸康复训练的基础上,开展说话瓣膜训练可有效提升脑卒中气管切开患者的呼吸肌功能及咳嗽效能.

Objective:To investigate the effect and safety of speaking valve training on respiratory muscle function and cough efficacy in stroke patients with tracheostomy.Methods:A total of 100 stroke patients with tracheostomy who were hospitalized in our hospital from June 2022 to February 2025 were selected and divided into an experimental group and a control group with 50 patients in each group by random number table method.The control group received conventional swallowing and breathing rehabilitation training,while the experimental group received additional speaking valve training on this basis.The changes in cough peak flow(CPF)and maximum inspiratory pressure(MIP)were detected in both groups before and after treatment.Results:After 3 weeks of treatment,both the experimental group and the control group showed significant improvement in MIP and CPF compared with those before treatment(P<0.05),and the improvement range of MIP and CPF in the experimental group was significantly greater than that in the control group(interaction effect P<0.05).Conclusion:On the basis of conventional swallowing and breathing rehabilitation training,speaking valve training can effectively improve the respiratory muscle function and cough efficacy of stroke patients with tracheostomy.
