目的:观察活络效灵丹对骨骼肌挫伤(Skeletal Muscle Contusion,SMC)大鼠患侧腿围、步态、组织形态学及炎症指标的影响,探讨不同浓度活络效灵丹对SMC的疗效.方法:将40只健康雄性SD大鼠常规喂养1周后,采用随机数字表法分为空白组、模型组、活络效灵丹低浓度组、活络效灵丹高浓度组和双氯芬酸钠组,每组各8只.除空白组外,其余4组SD大鼠均建立骨骼肌顿挫伤模型.造模前采用CatWalk运动步态分析系统采集5组大鼠步态数据.造模12 h后测量5组大鼠患侧腿围,造模24 h后开始给予药物干预,活络效灵丹低、高浓度组均按体质量10 mL/kg·d分别给予0.083 g/mL、0.332 g/mL活络效灵丹混悬液灌胃、双氯芬酸钠组按体质量10 mL/kg·d给予9 mg/mL双氯芬酸钠溶液灌胃,对照组和模型组每天给予等量生理盐水灌胃,每天2次,连续给药1周.干预1周后测量5组大鼠患侧腿围变化,采用步态分析系统采集各组大鼠步态数据,采用HE染色观察腓肠肌组织形态,采用酶联免疫吸附测定法(ELISA检测)测量腓肠肌组织肿瘤坏死因子α(TNF-α)及白细胞介素6(IL-6)含量.结果:与空白组比较,模型组腿围变化差值显著增加(P<0.01);平均摆动速度、平均站立时相显著减小(P<0.01);腓肠肌组织HE染色显示肌纤维排列紊乱、结构不清晰,细胞间隙增大,伴有大量红细胞及炎性细胞积聚;腓肠肌组织TNF-α及IL-6含量显著升高(P<0.01).与模型组比较,活络效灵丹低、高浓度组、双氯芬酸钠组患侧腿围变化差值显著降低(P<0.01);平均摆动速度、平均站立时相显著增加(P<0.01);腓肠肌组织HE染色显示肌纤维排列更为紧密、结构清晰度提升、细胞间隙减小、炎症细胞及红细胞聚集程度显著减轻;腓肠肌组织TNF-α及IL-6含量显著降低(P<0.01).与低浓度组比较,高浓度组患侧腿围变化差值降低(P<0.01);平均摆动速度、平均站立时相增加(P<0.01);腓肠肌组织HE染色显示肌纤维排列紧密、细胞间隙减小、未见炎症细胞及红细胞聚集;腓肠肌组织TNF-α及IL-6含量降低(P<0.01).结论:活络效灵丹可显著减轻骨骼肌损伤大鼠炎症反应,修复骨骼肌组织结构,提升大鼠运动能力;活络效灵丹对SMC疗效与浓度呈正相关可能.

Objective:To observe the effects of Huoluo Xiaoling Dan(HLXLD)on leg circumference,gait,histomorphology and inflammatory indicators in rats with skeletal muscle contusion(SMC),and to investigate the efficacy of different concentrations of HLXLD on SMC.Methods:Forty healthy male Sprague-Dawley(SD)rats were routinely fed for 1 week,and randomly divided into 5 groups(n=8 each):blank group,model group,low-concentration HLXLD group,high-concentration HLXLD group,and diclofenac sodium group.SMC model was established in all rats except the blank group.Gait data were collected using the CatWalk gait analysis system before modeling.The affected leg circumference was measured at 12 h after modeling.Drug intervention was initiated 24 h after modeling:rats in low-and high-concentration HLXLD groups were intragastrically administered HLXLD suspension at 0.083 g/mL and 0.332 g/mL,respectively;the diclofenac sodium group was given 9 mg/mL diclofenac sodium solution;blank and model groups received equal volume of normal saline.All interventions were given twice daily for 1 week at 10 mL/kg·d.After intervention,the affected leg circumference was measured,gait data were collected,gastrocnemius muscle morphology was observed by hematoxylin-eosin(HE)staining,and the levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were detected by enzyme-linked immunosorbent assay(ELISA).Results:Compared with the blank group,the model group showed significantly increased leg circumference difference(P<0.01),significantly decreased average swing speed and average standing phase(P<0.01),disorganized muscle fibers,enlarged intercellular space,massive erythrocyte and inflammatory cell infiltration,as well as significantly elevated TNF-α and IL-6 levels(P<0.01).Compared with the model group,low-concentration HLXLD,high-concentration HLXLD and diclofenac sodium groups exhibited significantly reduced leg circumference difference(P<0.01),increased average swing speed and standing phase(P<0.01),alleviated muscle fiber disarrangement and inflammatory cell infiltration,and markedly decreased TNF-α and IL-6 levels(P<0.01).Compared with the low-concentration HLXLD group,the high-concentration HLXLD group showed further decreased leg circumference difference(P<0.01),increased swing speed and standing phase(P<0.01),tighter muscle fiber arrangement,smaller intercellular space,less inflammatory cell and erythrocyte aggregation,and lower TNF-α and IL-6 levels(P<0.01).Conclusion:HLXLD can significantly alleviate inflammation,repair skeletal muscle structure,and improve motor function in SMC rats.The therapeutic effect of HLXLD on SMC may be positively correlated with its concentration.
