目的:观察电针八髎穴联合盆底肌生物反馈训练对脊髓损伤患者神经源性肠道功能的影响.方法:纳入脊髓损伤后神经源性肠道功能障碍患者40例,随机分为对照组与观察组,每组20例.对照组给予日常饮食调理、常规康复治疗及盆底肌生物反馈训练,观察组在此基础上联合电针八髎穴治疗,治疗5 d/周,疗程为8周.分别于治疗前及完成疗程后,通过肛门直肠测压检查(包括直肠静息压、肛管静息压、肛管舒张压、肛管括约肌功能长度、肛管最大收缩压及最长收缩时间)、NBD评分、便秘患者症状自评量表评分(PAC-SYM)及乳果糖-开塞露用量,对两组患者治疗前后的神经源性肠道功能改善情况进行疗效分析.结果:治疗后,两组患者的直肠测压指标RAP、ACRP、FACL、NBD评分及PAC-SYM均较治疗前明显降低,差异具有统计学意义(P<0.05),乳果糖及开塞露平均用量均较治疗前有明显减少,差异具有统计学意义(P<0.05),RRP、MSP与LSD较治疗前明显升高,差异具有统计学意义(P<0.05).其中观察组的肛门直肠测压指标RAP、ACRP、FACL及NBD评分、PAC-SYM评分、乳果糖和开塞露用量均较对照组明显减少,差异具有统计学意义(P<0.05),RRP、MSP与LSD较对照组明显升高,差异具有统计学意义(P<0.05).结论:电针八髎穴联合盆底肌生物反馈训练能有效改善脊髓损伤后神经源性肠道功能障碍患者的排便功能.

Objective:To observe the effects of electro-needling Baliao points combined with pelvic floor muscle biofeedback training on neurogenic bowel function in patients with spinal cord injury(SCI).Methods:A total of 40 patients with neurogenic bowel dysfunction(NBD)following SCI were randomly assigned into the control group and the observation group.The control group received routine dietary management,conventional rehabilitation therapy,and pelvic floor muscle biofeedback training;on which basis,the observation group was also treated with electro-needling Baliao points.Treatment was administered five days per week for eight weeks.Before and after the treatment,neurogenic bowel function was evaluated using anorectal manometry,including resting rectal pressure(RRP),anal canal resting pressure(ACRP),resting anal pressure(RAP),functional anal canal length(FACL),maximum squeeze pressure(MSP),and longest squeeze duration(LSD),as well as NBD score,patient assessment of constipation symptoms(PAC-SYM)score,and lactulose-glycerin enema usage.Results:The rectal manometry parameters RAP,ACRP,and FACL,as well as NBD scores and PAC-SYM scores were significantly decreased after the treatment compared with those before the treatment(P<0.05).The average usage of lactulose and glycerin enemas was also markedly reduced(P<0.05).In contrast,RRP,MSP,and LSD were significantly increased compared with pre-treatment values(P<0.05).The observation group demonstrated more significant improvements than the control group(P<0.05).Conclusion:Electro-needling Baliao acupoints combined with pelvic floor muscle biofeedback training can effectively improve the defecation function of patients with neurogenic intestinal dysfunction after SCI.
