基于"因虚致病"理论和孟德尔随机化探讨虚弱与肝酶相关性OA
Exploration on the Correlation between Frailty and Liver Enzymes Based on the Theory of"Illness Caused by Deficiency"and Mendelian Randomization
目的 基于中医"因虚致病"理论和孟德尔随机化探讨虚弱与肝酶水平之间的因果关系.方法 利用全基因组关联研究(GWAS)数据库数据,确定与虚弱和肝酶水平相关的特定单核苷酸多态性(SNPs),将SNPs作为工具变量,采用逆方差加权法(IVW)、MR-Egger回归法、加权中位数法3种分析方法进行孟德尔随机化分析,评估虚弱与肝酶水平的相关性.同时,为确保研究结果的可靠性和稳健性,进行敏感性分析、多效性检验,包括Cochran's Q检验、MR-Egger检验、MR-PRESSO检验和留一法分析.结果 UKB数据库分析显示,虚弱与丙氨酸氨基转移酶之间存在明显的正向因果关系(Beta=0.095,95%CI为0.036~0.153,P=0.001).虚弱与碱性磷酸酶(Beta=0.144,95%CI为0.054~0.235,P=0.002)、虚弱与γ-谷氨酰转移酶(GGT)(Beta=0.157,95%CI为0.072~0.242,P<0.001)及虚弱与天冬氨酸氨基转移酶(Beta=0.092,95%CI为0.023~0.160,P=0.008)也存在明显相关性.此外,GWAS Catalog数据库数据计算结果支持虚弱与丙氨酸氨基转移酶、碱性磷酸酶和γ-谷氨酰转移酶之间存在正向因果联系(P<0.05).结论 虚弱与肝酶水平存在明显相关性,可丰富"因虚致病"理论生物学内涵,同时提示临床上应将虚弱视为肝酶水平升高的潜在风险因素.
Objective To investigate the causal relationship between frailty and liver enzyme levels based on the theory of"illness caused by deficiency"in TCM and Mendelian randomization.Methods Using data from genome-wide association studies(GWAS)databases,specific single nucleotide polymorphisms(SNPs)related to frailty and liver enzyme levels were identified and used as instrumental variables.Mendelian randomization analysis was conducted using three methods:inverse variance weighted(IVW),MR-Egger regression,and weighted median,to evaluate the correlation between frailty and liver enzyme levels.Sensitivity analysis and pleiotropy tests were performed,including Cochran's Q test,MR-Egger test,MR-PRESSO test,and leave-one-out analysis,in order to ensure the reliability and robustness of the results.Results After analyzing data from the UK Biobank(UKB)database,a significant positive causal relationship was found between frailty and ALT(Beta=0.095,95%CI:0.036-0.153,P=0.001).The significant correlation was identified between frailty and ALP(Beta=0.144,95%CI:0.054-0.235,P=0.002),frailty and GGT(Beta=0.157,95%CI:0.072-0.242,P<0.001),and frailty and AST(Beta=0.092,95%CI:0.023-0.160,P=0.008).Furthermore,results from the GWAS Catalog database also supported the presence of a positive causal relationship between frailty and ALT,ALP,and GGT(P<0.05).Conclusion There is a significant correlation between frailty and liver enzymes,enriching the biological connotation of the theory of"illness caused by deficiency".At the same time,it is suggested that frailty should be considered as a potential risk factor for elevated liver enzyme levels in clinical practice.
