基于隐结构模型和关联规则分析梅尼埃病方药规律OA
Analysis on Prescription Law of for Ménière's Disease Based on Latent Structure Models and Association Rule Analysis
目的 探讨中医治疗梅尼埃病的方药规律,为临床提供参考.方法 检索中国知网(CNKI)、万方数据知识服务平台(Wangfang Data)、维普中文期刊服务平台(VIP)、中国生物医学文献服务系统(SinoMed)、PubMed、Web of Science、Cochrane Library、Embase建库至2025年3月4日收录的中医药治疗梅尼埃病相关文献,提取方药数据后运用Excel2019统计中药使用频次、性味归经及功效类别,并采用Lantern5.0和RStudio对使用频率≥3%的高频中药进行隐结构建模、聚类分析及关联规则分析.结果 共纳入397篇文献,提取441首处方,涉及222味中药,累计使用4 848次.高频中药为白术、茯苓、泽泻、天麻等;药性以温、平、寒为主,药味多为甘、辛、苦,主要归脾、肺、胃经;功效以补虚、利水渗湿、平肝息风为主.隐结构分析得到21个隐变量、42个隐类及7个聚类模型,归纳出25个核心方剂,推断主要证型为痰湿中阻、肾精不足、肝阳上亢、气血亏虚.关联规则分析得到47条强关联规则,其中支持度最高的组合为茯苓→白术,置信度最高的组合为白术+陈皮+甘草→茯苓.结论 梅尼埃病以肝肾亏虚、气血亏虚、髓海不足为本,风、火、痰、瘀扰窍为标,治疗应补虚泻实、调和阴阳,配合化痰祛湿、行气通滞、滋养肝肾及平肝潜阳等治法.
Objective To explore the prescription law of TCM treatment for Ménière's disease;To provide clinical references.Methods Relevant literature about TCM treatment for Ménière's disease was retrieved from CNKI,Wanfang Data,VIP,SinoMed,PubMed,Web of Science,Cochrane Library,Embase from the establishment of the databases to March 4,2025.Data analysis was conducted on frequency,properties and tastes,meridian tropism and efficacy of Chinese materia medica using Excel 2019.Lantern 5.0 and RStudio were used to perform latent structure modeling,clustering analysis,and association rule analysis on high-frequency Chinese materia medica with a usage frequency of≥3%.Results A total of 397 articles were included,including 441 prescriptions,which involving 222 herbs,with a total frequency of 4 848 times.High-frequency Chinese materia medica included Atractylodis Macrocephalae Rhizoma,Poria,Alismatis Rhizoma and Gastrodiae Rhizoma.The main properties were warm,neutral and cold;the main tastes were sweet,pungent and bitter,mainly entering the spleen,lung and stomach meridians.Key efficacy included tonifying deficiency,promoting diuresis and draining dampness,and calming the liver and extinguishing wind.Latent structure analysis revealed 21 latent variables,42 latent classes,and 7 cluster models,summarizing 25 core formulas.The main syndrome patterns identified were phlegm-dampness obstruction,kidney essence deficiency,liver yang hyperactivity,and qi-blood deficiency.Association rule analysis yielded 47 strong rules;the highest support was Poria → Atractylodis Macrocephalae Rhizoma,and the highest confidence was Atractylodis Macrocephalae Rhizoma+Citri Reticulatae Pericarpium+Glycyrrhizae Radix et Rhizoma → Poria.Conclusion Ménière's disease is characterized by liver-kidney deficiency,qi-blood deficiency,and marrow sea insufficiency in root,with manifestations of wind,fire,phlegm and stasis disturbing the orifices.Treatment should focus on tonifying deficiency and draining excess,harmonizing yin and yang,combined with method of resolving phlegm,dispelling dampness,regulating qi,unblocking stagnation,nourishing the liver and kidney,and calming the liver and subduing yang.
