Exploring the Selection Patterns of Acupuncture Points for Optic Atrophy Treatment Based on Data Mining

目的 基于数据挖掘技术探析针刺治疗视神经萎缩的临床选穴规律.方法 检索建库至2024年10月31日中国知网、万方和维普数据库中关于针刺治疗视神经萎缩的临床研究类文献,将纳入文献的发表时间、题目、作者、腧穴处方录入Excel 2016软件建立数据库,将腧穴按归经进行分类统计,记录其使用频次.采用IBM SPSS Modeler 18.0软件分析腧穴的关联规则,使用Origin 2021软件的聚类分析功能,对高频腧穴进行可视化多维度分析,并生成谱系图.结果 (1)一般情况:共纳入文献203篇.涉及经脉12条,穴位104个,累计使用频次为2,816次,使用频次≥22次的腧穴为高频腧穴,共24个.(2)经脉统计:针刺治疗视神经萎缩高频腧穴≥3个的经脉有3条,分别为足太阳膀胱经、足少阳胆经、足阳明胃经.高频腧穴使用频次≥100次的经脉有7条,排在前5位的是足太阳膀胱经(451次,24.95%)、足阳明胃经(270次,14.97%)、经外奇穴(237次,13.11%)、足少阳胆经(211次,11.67%)、足少阴肾经(142次,7.85%).(3)高频腧穴频次统计:使用频次排在前5位依次为睛明(152次,8.41%)、风池(145次,8.02%)、球后(126次,6.97%)、承泣(115次,6.36%)、足三里(115次,6.36%).(4)高频腧穴关联规则:相关性最高的前5位是睛明-承泣、睛明-足三里、足三里-三阴交、睛明-三阴交、肾俞-肝俞.(5)高频腧穴聚类分析:可得到6组核心处方.其功用分别为上注精、气、血于目,通调眼周气血经络,通阳明目,疏肝理气,活血祛瘀,镇静止痛.结论 针刺治疗视神经萎缩主要采用远近配穴法为主的取穴原则,多取穴三阳经,目系与全身经脉合治,治法以补气血、益肝肾、通络明目为主.

OBJECTIVE To analyze the clinical acupoint selection patterns in acupuncture treatment for optic nerve atrophy based on data mining technology.METHODS Clinical research literature on acupuncture treatment for optic nerve atrophy was retrieved from the China National Knowledge Infrastructure(CNKI),Wanfang,and VIP databases up to October 31st,2024.The publication time,title,author,and acupoint prescriptions of the included literature were entered into Excel 2016 software to establish a database.Acupoints were classified and counted according to the meridians they belong to,and their usage frequency was recorded.IBM SPSS Modeler 18.0 software was used to analyze association rules of acupoints,and the clustering analysis function of Origin 2021 software was employed to perform visual multidimensional analysis on high-frequency acupoints,generating a phylogenetic tree.RESULTS(1)General information:A total of 203 articles were included.The analysis involved 12 meridians and 104 acupoints,with a cumulative usage frequency of 2,816 times.Acupoints with a usage frequency of 22 times or more were considered high-frequency,totaling 24 acupoints.(2)Meridian statistics:Three meridians each contained three or more high-frequency acupoints used in acupuncture treatment for optic nerve atrophy.The Bladder Meridian of Foot-Taiyang,the Gallbladder Meridian of Foot-Shaoyang,and the Stomach Meridian of Foot-Yangming.Additionally,seven meridians had a usage frequency of 100 times or more.The top five were Bladder Meridian of Foot-Taiyang(451 times,24.95%),Stomach Meridian of Foot-Yangming(270 times,14.97%),extra points(237 times,13.11%),Gallbladder Meridian of Foot-Shaoyang(211 times,11.67%),and Kidney Meridian of Foot-Shaoyin(142 times,7.85%).(3)High-frequency acupoint frequency statistics:The top five acupoints by usage frequency were Jingming(BL1,152 times,8.41%),Fengchi(GB20,145 times,8.02%),Qiuhou(EX-HN7,126 times,6.97%),Chengqi(ST1,115 times,6.36%),and Zusanli(ST36,115 times,6.36%).(4)High-frequency acupoint association rules:The top five correlations with the highest confidence were Jingming(BL1)-Chengqi(ST1),Jingming(BL1)-Zusanli(ST36),Zusanli(ST36)-Sanyinjiao(SP6),Jingming(BL1)-Sanyinjiao(SP6),and Shenshu(BL23)-Ganshu(BL18).(5)High-frequency acupoint cluster analysis:Six core prescription groups were identified.Their functions were respectively Directing Jing(Essence),Qi,and blood upward to the eyes,regulating Qi,blood,and meridians around the eyes,clearing and regulating Yangming to brighten the eyes,soothing liver and regulating Qi,activating blood and dispelling stasis and tranquilizing and relieving pain.CONCLUSIONS The principle of acupoint selection in acupuncture treatment for optic nerve atrophy primarily employs the method of combining distal and local points.Points are frequently selected from the three Yang meridians,treating the eye system in conjunction with the whole body´s meridian system.The main treatment principles are tonifying Qi and blood,supplementing liver and kidney,unblocking collaterals,and brightening the eyes.
