基于数据挖掘分析周尚昆治疗干眼的用药规律OA

Data Mining-based Analysis of ZHOU Shangkun's Medication Patterns in Treating Dry Eye

目的 基于数据挖掘分析周尚昆治疗干眼的用药规律.方法 选取2018年1月—2023年12月中国中医科学院望京医院周尚昆干眼专病门诊治疗的干眼患者的病历,根据纳入与排除标准筛选,对病历信息进行结构化和规范化处理.将病历数据导入中医传承计算平台V3.0软件进行处理分析,使用Python 3.9.7软件进行频繁闭项集挖掘,再通过最小覆盖频繁项集压缩方法对结果进行精简,输出核心药物组合.结果 (1)一般情况:共收集得到干眼病历2,734份,筛选后最终纳入1,928份.抽取病历人工复核的症状识别准确率为91.3%,用药编码一致率为95.6%.(2)中药及其他治疗手段使用频次:中药处方共1,206个,包含242味中药,使用频次排名前3位的分别是菊花630次(52.23%)、蒺藜606次(50.25%)、薄荷588次(48.76%).其他治疗手段处方共722个,包含17种治疗手段,使用频次排名前3位的分别是结膜囊冲洗657次(52.23%)、脉冲治疗191次(26.45%)、中医定向透药治疗189次(26.18%).(3)用药规律分析:采用最小覆盖法得到256个频繁闭项集,可完全覆盖所有处方规律.进一步筛选出16个代表性高、非单一用药且差异比例较高的频繁闭项集,排名前2位的药物组合为①生地黄、川芎、当归、白芷、防风、蒺藜、赤芍、牡丹皮、薄荷;②玉竹、黄精、熟地黄、丹参、当归、薄荷.结论 周尚昆治疗干眼多从"滋阴养血、通络润目"出发,用药以滋阴药为主,配合通络药物,同时应用西医及中医适宜技术,为临床治疗干眼提供了可参考的客观依据.

OBJECTIVE To analyze the medication patterns of ZHOU Shangkun in treating dry eye based on data mining.METHODS Medical records of dry eye patients treated at ZHOU Shangkun´s specialized dry eye clinic at Wangjing Hospital,China Academy of Chinese Medical Sciences,from January 2018 to December 2023 were collected.Records were screened according to inclusion and exclusion criteria,and the information was structured and standardized.The data were imported into the Traditional Chinese Medicine(TCM)Inheritance Computing Platform V3.0 for processing and analysis.Python 3.9.7 was used for closed frequent itemset mining,and the results were refined using the minimum coverage frequent itemset compression method to output core medicine combinations.RESULTS(1)General information:A total of 2,734 dry eye medical records were collected,and 1,928 were included after screening.The manual review accuracy for symptom extraction was 91.3%,and the consistency rate for medication coding was 95.6%.(2)Frequency of Chinese medicine and other treatments:There were 1,206 medicine prescriptions involving 242 medicines.The top three most frequently used medicines were Chrysanthemi flos(630 times,52.23%),Tribuli fructus(606 times,50.25%),and Menthae haplocalycis herba(588 times,48.76%).There were 722 prescriptions for other treatments involving 17 modalities.The top three most frequently used treatments were conjunctival sac irrigation(657 times,52.23%),pulsed therapy(191 times,26.45%),and TCM directional medicine penetration therapy(189 times,26.18%).(3)Analysis of medication patterns:A total of 256 closed frequent itemsets were obtained using the minimum coverage method,which completely covered all prescription patterns.After further screening,16 representative closed frequent itemsets with high frequency,non-single medicine composition,and high differentiation ratios were identified.The top two medicine combinations were:① Rehmanniae radix,Chuanxiong rhizoma,Angelicae sinensis radix,Angelicae dahuricae radix,Saposhnikoviae radix,Tribuli fructus,Paeoniae radix rubra,Moutan cortex,Menthae haplocalycis herba;② Polygonati odorati rhizoma,Polygonati rhizoma,Rehmanniae radix praeparata,Salvia miltiorrhiza bunge,Angelicae sinensis radix,Menthae haplocalycis herba.CONCLUSIONS ZHOU Shangkun´s treatment of dry eye is primarily based on the principles of"nourishing Yin and blood,unblocking collaterals,and moistening the eyes".Medicine prescriptions are dominated by Yin-nourishing medicines combined with collateral-unblocking agents.Western Medical and appropriate TCM techniques are also applied.This study provides an objective reference for the clinical treatment of dry eye.
