目的:探讨妊娠合并乙肝病毒(HBV)感染患者血清DNA甲基转移酶1(DNMT1)、干扰素γ诱导蛋白-10(IP-10)、IFN-γ表达与母婴不良结局的关系.方法:选取2022年1月至2025年1月于商丘市第一人民医院收治的妊娠合并HBV患者107例作为感染组,选取同期进行孕检及分娩的72例健康孕妇作为对照组,比较两组血清DNMT1、IP-10、IFN-γ水平及母婴不良结局情况.根据母婴不良结局发生情况将感染组分为母婴不良结局组(n=59)和母婴良好结局组(n=48),通过单变量及多变量Logistic回归模型筛选HBV感染孕妇出现母婴不良结局的独立危险因素,并应用受试者工作特征(ROC)曲线分析血清DNMT1、IP-10及IFN-γ浓度指标对HBV感染孕妇母婴不良结局的预测效能.结果:感染组DNMT1、IP-10水平显著高于对照组(P<0.05),IFN-γ水平显著低于对照组(P<0.05),且感染组妊娠期肝内胆汁淤积症、妊娠期糖尿病、新生儿窒息及早产的发生率显著高于对照组(P<0.05);与母婴良好结局组相比,母婴不良结局组HBV病毒载量阳性占比、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、DNMT1、IP-10水平显著升高,IFN-γ水平显著降低(P<0.05);多因素分析显示:HBV病毒载量≥1.0×103 U/mL、血清AST、ALT、TBIL、DNMT1、IP-10指标是妊娠合并HBV感染患者母婴不良结局的独立危险因素,IFN-γ是其保护因素(P<0.05);ROC结果显示:DNMT1、IP-10、IFN-γ单独及联合检测对于预测妊娠合并HBV感染患者母婴不良结局均具有统计学意义(P<0.05);其中联合检测的AUC为0.960,95%CI为0.929~0.990,灵敏度为0.831,特异度为0.958,预测价值较高.结论:妊娠合并HBV感染患者中,血清DNMT1与IP-10水平升高及IFN-γ水平降低与母婴不良结局存在显著关联.DNMT1和IP-10是此类患者发生母婴不良结局的独立危险因素,而IFN-γ则为保护因素,三者联合检测对母婴不良结局的预测效能优于单一指标.

Objective:To investigate the relationship between serum DNA methyltransferase 1(DNMT1),interferon gamma-inducible protein-10(IP-10),and IFN-γ expressions and adverse maternal-infant outcomes in pregnant women with hepatitis B virus(HBV)infection.Methods:A total of 107 pregnant women with HBV infection admitted to The First People's Hospital of Shangqiu from January 2022 to January 2025 were enrolled as the infected group,and 72 healthy pregnant women undergoing prenatal examina-tion and delivery during the same period were selected as the control group.Serum levels of DNMT1,IP-10,and IFN-γ,as well as the incidence of adverse maternal-infant outcomes,were compared between the two groups.According to pregnancy outcomes,the infect-ed group was further divided into an adverse maternal-infant outcome group(n=59)and a favorable maternal-infant outcome group(n=48).Univariate and multivariate Logistic regression models were used to identify independent risk factors for adverse maternal-infant outcomes.Receiver operating characteristic(ROC)curve analysis was performed to evaluate the predictive efficiency of serum DN-MT1,IP-10,and IFN-γ for adverse maternal-infant outcomes in pregnant women with HBV infection.Results:DNMT1 and IP-10 lev-els were significantly higher in the infected group than those in the control group(P<0.05),while IFN-γ level was significantly lower than in the control group(P<0.05).The incidence rates of intrahepatic cholestasis of pregnancy,gestational diabetes mellitus,neona-tal asphyxia,and preterm birth were also significantly higher in the infected group(P<0.05).Compared with the favorable outcome group,the adverse outcome group exhibited significantly higher positive rate of HBV viral load,levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),DNMT1,and IP-10,as well as significantly lower level of IFN-γ(P<0.05).Multivariate analysis showed that HBV viral load≥1.0×10³ U/mL,elevated serum AST,ALT,TBIL,DNMT1,and IP-10 were independent risk factors for adverse maternal-infant outcomes,while IFN-γ was a protective factor(P<0.05).ROC curve analysis demonstrated that DNMT1,IP-10,IFN-γ alone and in combination all showed statistically significant predictive value for adverse maternal-infant outcomes(P<0.05).The combined detection achieved an AUC of 0.960(95%CI:0.929~0.990),with a sensitivity of 0.831 and a specificity of 0.958,indicating high predictive performance.Conclusion:Increased serum levels of DNMT1 and IP-10 and decreased level of IFN-γ are significantly associated with adverse maternal-infant outcomes in pregnant women with HBV infec-tion.DNMT1 and IP-10 are independent risk factors for adverse maternal-infant outcomes,whereas IFN-γ serves as a protective factor.The combined detection of the three indicators exhibits better predictive efficiency than any single indicator.
