METTL3、IREB2在非小细胞肺癌组织中的表达及意义OA
Expression and significance of METTL3 and IREB2 in non-small cell lung cancer tissues
目的 探讨甲基转移酶样蛋白3(METTL3)、铁反应元件结合蛋白2(IREB2)在非小细胞肺癌(NSCLC)组织中的表达及其临床意义.方法 本研究为回顾性队列研究,纳入2021年1月至2023年3月无锡市第二人民医院收治的102例NSCLC患者为研究对象,另选取100例肺部良性病变患者作为正常对照.RT-PCR法检测正常肺组织、癌组织及癌旁组织中METTL3、IREB2的表达水平.收集所有患者临床资料,分析METTL3、IREB2与NSCLC患者临床病理特征的关系;依据随访情况,将NSCLC患者分为生存组和死亡组,单因素分析及多因素Cox回归分析影响NSCLC患者预后的因素;绘制受试者工作特征(ROC)曲线,评估METTL3、IREB2对NSCLC患者预后的预测价值.结果 NSCLC患者癌组织中METTL3的表达水平高于癌旁组织,而IREB2的表达水平低于癌旁组织,差异均具有统计学意义(P<0.001).与TNM分期Ⅰ~Ⅱ期和无淋巴结转移患者相比,Ⅲ~Ⅳ期和淋巴结转移患者的癌组织中METTL3表达水平显著升高,IREB2表达显著降低,差异均具有统计学意义(P<0.05).多因素Cox回归分析结果显示,TNM分期Ⅲ~Ⅳ期、淋巴结转移、METTL3高表达及IREB2低表达均是影响NSCLC患者预后的独立危险因素(P<0.05).ROC曲线分析结果显示,NSCLC患者癌组织中METTL3、IREB2联合预测预后的曲线下面积(AUC)为0.865,敏感度89.30%,特异度78.36%,均高于二者单独预测.结论 NSCLC患者癌组织中METTL3高表达、IREB2低表达与临床病理特征密切相关,是导致患者不良预后的独立危险因素,且二者联合对NSCLC患者不良预后具有较高的预测价值.
Objective To investigate the expression and clinical significance of methyltransferase-like protein 3(METTL3)and iron responsive element binding protein 2(IREB2)in non-small cell lung cancer(NSCLC)tissues.Methods This retrospective cohort study enrolled 102 NSCLC patients admitted to Wuxi Second People's Hospital from January 2021 to March 2023,and 100 patients with benign lung lesions served as the normal control.The expression levels of METTL3 and IREB2 in normal lung tissues,tumor tissues,and paracan-cerous tissues were detected by RT-PCR.The clinical data of all patients were collected,and the relationships of METTL3 and IREB2 expression with the clinicopathological features of NSCLC patients were analyzed.Based on the follow-up status,NSCLC patients were divided into the survival group and the death group.Univariate analysis and multivariate Cox regression analysis were conducted to analyze the factors affecting the prognosis of NSCLC patients.Receiver operating characteristic(ROC)curve was plotted to evaluate the predictive value of METTL3 and IREB2 for the prognosis in NSCLC patients.Results The expression level of METTL3 in tumor tissues of NSCLC patients was higher than that in paracancerous tissues,while the expression level of IREB2 was lower than that in paracancerous tissues,with statistically significant differences(P<0.001).Compared with patients at TNM stages Ⅰto Ⅱ or those without lymph node metastasis,the expression level of METTL3 was significantly increased and IREB2 was significantly decreased in cancer tissues of patients at stages ⅢtoⅣ or those with lymph node metastasis,with statistically significant differences(P<0.05).Multivariate Cox regression analysis results indicated that TNM stages Ⅲ to Ⅳ,lymph node metastasis,high METTL3 expression,and low IREB2 expression were all the independent risk factors for the poor prognosis in NSCLC patients(P<0.05).ROC curve analysis showed that the combined use of METTL3 and IREB2 expression in cancer tissues predicted the prognosis of NSCLC patients with an area under the curve of 0.865,a sensitivity of 89.30%,and a specificity of 78.36%,all of which were higher than those of either marker alone.Conclusion The high expression of METTL3 and low expression of IREB2 in tumor tissues of NSCLC patients are closely associated with clinicopathological features,and serve as independent risk factors for poor prognosis of patients.Furthermore,the combination of these two markers demonstrates high predictive value for poor prognosis in NSCLC patients.
