黄芪建中汤合小承气汤调控术后肠梗阻大鼠肌层巨噬细胞极化对Cajal间质细胞的影响OA
Huangqi Jianzhong Decoction combined with Xiao Chengqi Decoction ameliorates postoperative ileus in rats via regulating muscularis macrophage polarization to increase interstitial cells of Cajal
目的 探讨黄芪建中汤合小承气汤调节术后肠梗阻大鼠肌层巨噬细胞(MMs)极化影响Cajal间质细胞(ICC)的机制.方法 将 24 只SPF级雄性Wistar大鼠随机分为正常组、模型组、肥皂水组、黄芪建中汤合小承气汤组,每组6 只.除正常组外,其余组大鼠采用肠管表面擦拭法建立术后肠梗阻模型.术后第 1 天开始,黄芪建中汤合小承气汤组给予黄芪建中汤合小承气汤8.45 g/(kg·d)灌肠,肥皂水组给予等体积肥皂水灌肠,均1 次/d,持续1 周.干预结束后,测量小肠推进率,ELISA法检测小肠组织中MMs表面标志物CD86、CD206 及炎性因子肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)含量,Western blot法与免疫荧光法检测小肠组织中ICC标记物c-Kit蛋白表达情况.结果 与正常组比较,模型组大鼠肠壁水肿、粘连及炎症浸润明显;小肠推进率明显降低(P<0.05);小肠组织中CD86、CD206、TNF-α含量均明显升高(P均<0.05),IL-10含量、c-Kit蛋白相对表达量、c-Kit蛋白阳性表达平均光密度值均明显降低(P均<0.05).与模型组比较,肥皂水组和黄芪建中汤合小承气汤组大鼠肠壁水肿、粘连及炎症浸润减轻;黄芪建中汤合小承气汤组大鼠小肠推进率明显提高(P<0.05),小肠组织中CD206、TNF-α含量均明显降低(P均<0.05),小肠组织中IL-10 含量、c-Kit蛋白相对表达量、c-Kit蛋白阳性表达平均光密度值均明显升高(P均<0.05);肥皂水组小肠组织中CD86、CD206、TNF-α含量均明显降低(P均<0.05),小肠组织中IL-10 含量明显升高(P<0.05).黄芪建中汤合小承气汤组大鼠小肠推进率和小肠组织中CD86、CD206、IL-10 含量均明显高于肥皂水组(P均<0.05).结论 黄芪建中汤合小承气汤可明显减轻术后肠梗阻大鼠肠道损伤,促进肠道动力恢复,机制可能与其调节MMs向M2 型极化、抑制炎性反应、增加ICC数量有关.
Objective It is to investigate the mechanism of Huangqi Jianzhong Decoction combined with Xiaochengqi Decoction(HQJZ-XCQ)in affecting interstitial cells of Cajal(ICC)in rats with postoperative ileus(POI)via regulating muscularis macrophage(MMs)polarization.Methods Twenty-four SPF male Wistar rats were randomly divided into 4r groups:normal group,model group,soap solution group and HQJZ-XCQ group,with 6 rats in each group.The rats of all groups except for the normal group were prepared with intestinal surface abrasion method to establish models of POI.Start-ing from the first postoperative day,the HQJZ-XCQ group was given HQJZ-XCQ group 8.45 g/(kg·d)by enema,the soap solution group was given an equal volume of soap solution by enema,both administered once daily,continuously trea-ted for one week.At the end of treatment,the small intestinal propulsion rate was measured,the contents of MMC surface markers CD86 and CD206,as well as inflammatory factors tumor necrosis factor-α(TNF-α)and interleukin-10(IL-10)in small intestinal tissue were determined by ELISA,the protein expression of the ICC marker c-Kit protein in small intestinal tissue was detected by Western blot and immunofluorescence assays.Results Compared with the normal group,obvious e-dema,adhesions,and inflammatory infiltration were found in intestinal wall of rats in the model group;the small intestinal propulsion rate of the rats was significantly reduced(P<0.05);the contents of CD86,CD206 and TNF-α in small intesti-nal tissue were significantly increased(all P<0.05),while the content of IL-10,the relative protein expression of c-Kit,and the mean optical density value of positive protein expression of c-Kit were significantly reduced(all P<0.05).Com-pared with the model group,the edema,adhesions,and inflammatory infiltration in intestinal wall of rats in the soap solu-tion group and HQJZ-XCQ group were alleviated;the small intestinal propulsion rate of rats in the HQJZ-XCQ group was significantly increased(P<0.05),the contents of CD206 and TNF-α in small intestinal tissue were significantly decreased(all P<0.05),while the content of IL-10,the relative protein expression of c-Kit,and the mean optical density value of positive protein expression of c-Kit were significantly increased(all P<0.05);the contents of CD86,CD206 and TNF-α in small intestinal tissue of rats in the soap solution group were significantly decreased(all P<0.05),while the content of IL-10 was significantly increased(P<0.05).The small intestinal propulsion rate and the contents of CD86,CD206 and IL-10 in small intestinal tissue of rats in the HQJZ-XCQ group were significantly higher than those in the soap solution group.Conclusion Huangqi Jianzhong Decoction combined with Xiaochengqi Decoction can obviously alleviate intestinal injury and promote motility recovery in POI rats.The mechanism may involve the modulation of MMs polarization towards the M2 phenotype,inhibition of inflammatory responses,and reduction of ICC apoptosis.
