目的 基于Toll样受体4(TLR4)/核因子-κB(NF-κB)信号通路调控炎症反应探讨柴桂剂对高血脂症脑缺血再灌注损伤大鼠的神经保护作用及其机制.方法 取 120 只雄性大鼠,随机选择14 只作为正常组,其余大鼠采用高脂饲料喂养6 周建立高血脂症模型.然后将84 只造模成功高血脂症模型大鼠随机分为6 组,每组14 只.柴桂剂组、尼莫地平组、大柴胡汤组、桂枝茯苓丸组分别按照6.48 g/kg、20 mg/kg、4.05 g/kg、3.04 g/kg剂量灌胃相应药物,假手术组和模型组灌胃等量生理盐水,均1 次/d.灌胃第7 天,除假手术组外,其余组大鼠采用线栓法建立脑缺血再灌注损伤模型,缺血再灌注6h后各组给予相应药物或生理盐水再灌胃1 次.缺血再灌注24h后记录大鼠神经功能缺损评分,然后麻醉大鼠取血,生化分析仪检测血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平,酶联免疫吸附法测定血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、TLR4 水平;取大鼠脑组织,HE染色观察脑组织病理形态,免疫组化法检测缺血侧脑组织中TLR4、NF-κBp65 阳性表达情况,实时荧光定量和Western blot法检测缺血侧脑组织及结肠组织中TLR4、NF-κBp65、IL-1β mRNA和蛋白表达情况.结果 各给药组大鼠神经功能缺损评分均明显低于模型组(P均<0.05),各给药组间比较差异均无统计学意义(P均>0.05).与正常组比较,假手术组、模型组和各给药组大鼠血清TC、TG、LDL-C水平均明显升高(P均<0.05),血清HDL-C水平均明显降低(P均<0.05);与模型组和尼莫地平组比较,中药各组大鼠血清TC、TG、LDL-C水平均明显降低(P均<0.05),血清HDL-C水平均明显升高(P均<0.05);与大柴胡汤组和桂枝茯苓丸组比较,柴桂剂组大鼠血清TC、TG、LDL-C水平均更低(P均<0.05),血清HDL-C水平更高(P均<0.05).与假手术组比较,模型组大鼠脑组织损伤明显;血清TNF-α、IL-1β、TLR4 水平,缺血侧脑组织中TLR4、NF-κB p65 阳性表达平均光密度值,缺血侧脑组织及结肠组织中TLR4、NF-κBp65、IL-1β mRNA和蛋白相对表达量均明显升高(P均<0.05).与模型组比较,柴桂剂组和尼莫地平组脑组织损伤明显减轻;各给药组大鼠血清TNF-α、IL-1β、TLR4 水平,缺血侧脑组织中TLR4、NF-κBp65 阳性表达平均光密度值,缺血侧脑组织及结肠组织中TLR4、NF-κBp65、IL-1β mRNA和蛋白相对表达量均明显降低(P均<0.05),且柴桂剂组和尼莫地平组上述各指标均明显低于大柴胡汤组和桂枝茯苓丸组(P均<0.05).结论 柴桂剂可明显改善高血脂症脑缺血再灌注损伤大鼠神经功能,调节血脂水平,其可能通过抑制TLR4/NF-κB信号通路,减轻炎症反应,调节肠-脑轴平衡而发挥保护作用.

Objective It is to explore the neuroprotective effect and its potential mechanisms of Chai Gui Decoction in rats with hyperlipidemia and brain ischemia/reperfusion injury based on Toll-like receptor 4(TLR4)/nuclear factor-κB(NF-κB)signaling pathway.Methods One hundred and twenty male rats were taken,14 of which were randomly selected as normal group,and the remaining rats were fed with high-fat diet to establish models of hyperlipidemia.84 successfully modeled rats with hyperlipidemia were randomly divided into 6 groups,with 14 rats in each group.The Chai Gui Decoction group,nimodipine group,Da Chai Hu Decoction group and Guizhi Fuling Pill group were administered corresponding drugs via gastric gavage at doses of 6.48 g/kg,20 mg/kg,4.05 g/kg,and 3.04 g/kg,respectively,the sham operation group and model group were administered equal volumes of physiological saline via gastric gavage,all administered once daily.On the 7th day of gavage,the rats of all groups except for the sham operation group were prepared with suture-occluded method to establish models of cerebral ischemia-reperfusion injury.At 6 hours after ischemia-reperfusion,each group was administered corresponding drugs or physiological saline via gastric gavage once.At 24 hours after ischemia-reperfusion,the neurological dysfunction scores of the rats were recorded,then the rats were anesthetized for blood collection.The ser-um levels of total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C)and high-density lip-oprotein cholesterol(HDL-C)were measured by biochemical analyzers,the serum levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and TLR4 were measured by ELISA.The cerebral tissue of the rats was taken to observe the pathological morphology by HE staining,the positive expressions of TLR4 and NF-κB p65 in cerebral tissue of the ischemic side were detected by immunohistochemistry,the mRNA and protein expressions of TLR4,NF-κB p65 and IL-1β in cere-bral tissue of the ischemic side and colon tissues were detected by Real-time quantitative PCR and Western blot analysis,respectively.Results The neurological dysfunction scores of rats in all medicated groups were significantly lower than that in the model group(all P<0.05),but no statistically significant differences were observed among the medicated groups(all P>0.05).Compared with the normal group,the serum levels of TC,TG and LDL-C were significantly increased(all P<0.05),while the serum levels of HDL-C were significantly reduced in rats of the sham operation group,model group and all medicated groups(all P<0.05).Compared with the model group and nimodipine group,the serum levels of TC,TG and LDL-C were significantly reduced(all P<0.05),while the serum levels of HDL-C were significantly increased in rats of the traditional Chinese medicine treated groups(all P<0.05).Compared with the Da Chai Hu Decoction group and Guizhi Fuling Pill group,the serum levels of TC,TG and LDL-C were more lower(all P<0.05),while the serum level of HDL-C was more higher in rats of the Chai Gui Decoction group(P<0.05).Compared with the sham operation group,the brain tissue damage was obvious in rats of the model group;the serum levels of TNF-α,IL-1β and TLR4,the mean optical density values of positive expressions of TLR4 and NF-κB p65 in cerebral tissue of the ischemic side,and the mRNA and protein expressions of TLR4,NF-κB p65 and IL-1β in cerebral tissue of the ischemic side and colon tissues were signifi-cantly increased(all P<0.05).Compared with the model group,the brain tissue damage was obviously alleviated in rats of the Chai Gui Decoction group and nimodipine group;the serum levels of TNF-α,IL-1β and TLR4,the mean optical density values of positive expressions of TLR4 and NF-κB p65 in cerebral tissue of the ischemic side,and the mRNA and protein expressions of TLR4,NF-κB p65 and IL-1β in cerebral tissue of the ischemic side and colon tissues were signifi-cantly increased in rats of the medicated groups(all P<0.05),furthermore,these indicators were significantly lower in the Chai Gui Decoction group and nimodipine group than those in the Da Chai Hu Decoction group and Guizhi Fuling Pill group(all P<0.05).Conclusion Chai Gui Decoction can significantly improve neurological function and regulate blood lipid of rats with hyperlipidemia and cerebral ischemia-reperfusion injury,its protective effects may be achieved via inhibiting TLR4/NF-κB signaling pathway,reducing inflammatory responses,and regulating gut-brain axis balance.
