基于子午流注理论探究正常高值血压人群血压节律与时辰血压的特点OA
Characteristics of blood rhythm and two-hour period blood pressure in high-normal blood pressure population based on midnight-noon ebb-flow theory
目的 基于子午流注理论探究正常高值血压人群的血压节律与时辰血压的特点.方法 以2019 年9 月—2022 年9 月在北京中医药大学东直门医院及中国中医科学院广安门医院周边社区招募的247 例正常高值血压人群为研究对象,采集受试者基本信息并完善24h动态血压监测,对24h收缩压(SBP)峰值出现时间进行圆形分布分析,并比较不同血压节律者动态血压指标及时辰血压.结果 剔除资料不全及动态血压数据缺失者24 例,共223 例进入分析.SBP峰值集中在辰时至戌时(8:58:02-19:15:31).血压节律呈杓型43 例(杓型组),非杓型159 例(非杓型组),反杓型21 例(反杓型组).非杓型组及反杓型组受试者年龄均明显高于杓型组(P 均<0.05).夜间平均收缩压(nSBP)和夜间平均舒张压(nDBP)方面,反杓型组>非杓型组>杓型组,差异均有统计学意义(P 均<0.05);反杓型组受试者 24h 平均心率(24hHR)、白天平均心率(dHR)均明显慢于杓型组(P均<0.05).3 组间时辰SBP在亥时至辰时存在差异(P均<0.05),时辰DBP在子时至卯时存在差异(P均<0.05),反杓型组、非杓型组SBP、DBP均较高.结论 正常高值血压人群24hSBP峰值集中在辰时至戌时,且血压节律以非杓型为主,已存在血压节律异常现象;节律异常者亥时至辰时血压偏高,血压波动可能与时辰有关.
Objective It is to explore the characteristics of blood pressure rhythm and two-hour period blood pressure in the people with high-normal blood pressure based on midnight-noon ebb-flow theory.Methods A total of 247 individuals with high-normal blood pressure in the surrounding communities of Dongzhimen Hospital of Beijing University of Chinese Medicine and Guang'anmen Hospital of Chinese Academy of Traditional Chinese Medicine from September 2019 to Septem-ber 2022 were collected as the subjects.Their basic information was collected and 24-hour ambulatory blood pressure moni-toring was completed or supplied,the timing of peak systolic blood pressure(SBP)over 24 hours was analyzed with circu-lar distribution analysis,and dynamic blood pressure indicators and two-hour period SBP were compared among individuals with different blood pressure rhythms.Results Twenty-four cases with incomplete data or missing ambulatory blood pressure measurements were eliminated,and a total of 223 cases were included in the analysis.The peak SBP concentrated from Chen time to Xu time(8:58:02-19:15:31).Blood pressure rhythms were classified as dipper type in 43 cases(dipper group),non-dipper type in 159 cases(non-dipper group),and reverse dipper type in 21 cases(reverse dipper group).The age of subjects in both the non-dipper and reverse dipper groups was significantly higher than that in the dipper group(both P<0.05).Night mean systolic blood pressure(nSBP)and night mean diastolic blood pressure(nDBP)from high to low were as follows:reverse-dipper group>non-dipper group>dipper group,the differences were all statistically signifi-cant(all P<0.05);the24-hour mean heart rate(24hHR)and daytime mean heart rate(dHR)of subjects in the re-verse-dipper group were significantly slower than those of subjects in the dipper group(all P<0.05).There were signifi-cant differences in two-hour period SBP from Hai time to Chen time and in two-hour period DBP from Zi time to Mao time among the three groups(all P<0.05),and the SBP and DBP were higher in the reverse-dipper group and non-dipper group.Conclusion The peak value of 24-hour SBP in the high-normal blood pressure population is concentrated from Chen time to Xu time,and the blood pressure rhythm is mainly non-dipper pattern,already with abnormal blood pressure rhythm.The individuals with abnormal rhythms may have higher blood pressure from Hai time to Chen time,blood pressure fluctuations may be related to the two-hour periods.
