P波时限及P波离散度与房颤射频消融术后复发的关系探讨OA

目的 分析P波时限及P波离散度(Pd)与房颤(AF)射频消融术后复发的相关性.方法 选取本院 2023 年 9 月至2025 年 1 月收治的 100 例首次行射频消融术的AF患者,持续随访至术后 6 个月,根据患者术后 6 个月内AF复发情况进行分组,分为早期复发组(术后 3 个月内复发,n=24)、晚期复发组(术后 3~6 个月复发,n=18)及未复发组(术后 6 个月内未复发,n=58).比较三组P波时限(Pmin、Pmax)、Pd及心功能指标,并分析二者间的相关性.结果 早期复发组与晚期复发组Pmin均低于未复发组,Pmax、Pd均高于未复发组(P<0.05).早期复发组与晚期复发组Pmin、Pmax、Pd比较,差异无统计学意义(P>0.05).早期复发组与晚期复发组的左心房内径(LAD)高于未复发组,左室射血分数(LVEF)低于未复发组(P<0.05).早期复发组与晚期复发组LAD、LVEF比较,差异无统计学意义(P>0.05).Pearson相关性分析结果显示,Pmin与LAD呈负相关,与LVEF呈正相关(r=-0.321、0.335,P<0.05);Pmax、Pd 与LAD 呈正相关(r=0.249、0.435,P<0.05),与 LVEF 呈负相关(r=-0.298、-0.416,P<0.05).结论 射频消融术后AF复发患者Pmin更低,Pmax与Pd更高,且患者P波时限与Pd和心功能指标存在明显关联.临床可通过监测患者术后P波时限与Pd变化情况,预测疾病复发风险,尽早制定相关防范策略.

Objective To analyze the correlation between P-wave duration,P-wave dispersion(Pd)and recurrence after radiofrequency ablation for atrial fibrillation(AF).Methods 100 AF patients undergoing first-time radiofrequency ablation in our hospital from September 2023 to January 2025 were selected,and followed up continuously for 6 months after surgery.According to the recurrence of AF within 6 months after surgery,the patients were divided into early recurrence group(recurrence within 3 months after surgery,n=24),late recurrence group(recurrence between 3 and 6 months after surgery,n=18)and non-recurrence group(no recurrence within 6 months after surgery,n=58).The P-wave duration(Pmin,Pmax),Pd and cardiac function indicators were compared among the three groups,and the correlation between the above indicators was analyzed.Results The Pmin of the early recurrence group and late recurrence group were lower than those of the non-recurrence group,and the Pmax and Pd were higher than those of the non-recurrence group(P<0.05).No statistically significant difference was found in the Pmin,Pmax and Pd between the early recurrence group and the late recurrence group(P>0.05).The left atrial diameter(LAD)of the early recurrence group and late recurrence group were higher than those of the non-recurrence group,and the left ventricular ejection fraction(LVEF)were lower than those of the non-recurrence group(P<0.05).No statistically significant difference was found in the LAD and LVEF between the early recurrence group and the late recurrence group(P>0.05).Pearson correlation analysis results showed that Pmin was negatively correlated with LAD,and positively correlated with LVEF(r=-0.321,0.335,P<0.05);Pmax and Pd were positively correlated with LAD(r=0.249,0.435,P<0.05),and negatively correlated with LVEF(r=-0.298,0.416,P<0.05).Conclusions AF patients with recurrence after radiofrequency ablation have lower Pmin and higher Pmax and Pd.Moreover,there is a significant correlation between P-wave duration,Pd and cardiac function indicators.Clinically,monitoring the changes in P-wave duration and Pd after surgery can help predict the risk of disease recurrence and formulate preventive strategies as early as possible.
