目的:评估振幅整合脑电图(aEEG)评分联合血清肌酸激酶脑型同工酶(CK-BB)、视觉蛋白样蛋白1(VILIP-1)水平对早产儿脑损伤的预测价值.方法:选择90例脑损伤早产儿为脑损伤组,55例无脑损伤的早产儿为对照组.脑功能监护仪监测并获取早产儿 aEEG参数;酶联免疫法检测早产儿血清CK-BB、VILIP-1水平;Logis-tic回归分析早产儿发生脑损伤的影响因素;受试者工作特征(ROC)曲线评估 aEEG 评分及血清CK-BB、VILIP-1水平预测早产儿脑损伤的价值.结果:脑损伤组母亲合并妊娠期糖尿病及胎盘异常早产儿占比多于对照组(均P<0.05).相较于对照组,脑损伤组早产儿的aEEG连续性评分、睡眠-觉醒周期评分、下边界振幅评分以及总分明显降低,血清CK-BB、VILIP-1水平明显升高(均 P<0.05).早产儿母亲合并妊娠期糖尿病及早产儿血清CK-BB、VILIP-1水平升高是早产儿发生脑损伤的危险因素,aEEG 各参数评分升高是早产儿发生脑损伤的保护因素(均P<0.05).aEEG评分及血清CK-BB、VILIP-1 联合预测早产儿脑损伤的曲线下面积(AUC)明显高于各自单项指标(均P<0.05).结论:aEEG评分及血清CK-BB、VILIP-1对预测早产儿脑损伤有较高的临床应用价值,且三项联合的预测价值更高.

Objective:To evaluate the predictive value of amplitude-integrated electroencephalography(aEEG)score combined with serum Creatine kinase-BB(CK-BB)and visinin-like protein 1(VILIP-1)for brain injury in pre-mature infants.Methods:90 premature infants with brain injury were used as brain injury group,and 55 premature in-fants without brain injury were used as control group.The brain function monitor was used to monitor and obtain aEEG parameters of premature infants.Enzyme linked immunosorbent assay was used to detect serum CK-BB and VILIP-1 in premature infants.Logistic regression was used to analyze the influencing factors of brain injury in prema-ture infants.ROC curve was used to evaluate the value of aEEG score and serum CK-BB,VILIP-1 in predicting brain injury in premature infants.Results:The proportions of premature infants whose mothers complicated with gestation-al diabetes and placental abnormalities in brain injury group were higher than those in the control group(all P<0.05).Compared with control group,the aEEG continuity score,sleep-wake cycle score,lower boundary amplitude score,and total score of premature infants in the brain injury group were significantly reduced,while serum CK-BB and VILIP-1 levels were significantly increased(all P<0.05).Premature infants with mothers complicated with gesta-tional diabetes and elevated serum CK-BB and VILIP-1 were risk factors for brain injury,and the increase in parame-ter scores of aEEG were the protective factors(all P<0.05).The AUC of the combined prediction of serum CK-BB,VILIP-1,and aEEG score for brain injury in premature infants was significantly higher than that of each individual detection(all P<0.05).Conclusion:The aEEG score and serum CK-BB,VILIP-1 have high clinical application value in predicting brain injury in premature infants,and the predictive value of the three combined is higher.
