载木犀草素PMMA骨水泥对桡骨缺损大鼠成骨修复的影响OA
Effect of PMMA bone cement loaded with luteolin on osteogenic repair in rats with radial defects
目的:探究载木犀草素的聚甲基丙烯酸甲酯(PMMA)骨水泥对桡骨缺损大鼠成骨修复的影响.方法:构建桡骨缺损大鼠模型,将桡骨缺损模型大鼠分为对照组、PMMA骨水泥组和PMMA骨水泥+木犀草素组,每组18只.对照组大鼠骨缺损处不植入任何材料;PMMA骨水泥组大鼠骨缺损处植入不载木犀草素的PMMA骨水泥;PMMA骨水泥+木犀草素组植入载木犀草素的PMMA骨水泥.造模后第5周时,每组选取 6只大鼠在原桡骨切口处植入自体松质骨.实验期间观察大鼠的一般状态;X射线观察骨缺损区骨水泥轮廓、骨水泥与周围组织反应的范围及骨愈合情况;苏木素-伊红染色观察骨缺损区诱导膜及新生骨形态学变化,并评估新生骨占比及成骨细胞数;酶联免疫吸附法(ELISA)检测诱导膜组织中白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平;蛋白印迹法检测诱导膜组织中血管内皮生长因子(VEGF)、转化生长因子-β1(TGF-β1)和骨形态发生蛋白-2(BMP-2)蛋白表达.结果:实验期间所有动物食欲良好,体力活动良好,无死亡情况;术后1周,各组手术部位均可见水肿、充血、肿胀和疼痛.造模后第5周,与对照组比较,PMMA骨水泥组骨缺损区的骨水泥轮廓变得不规则,与周围组织界面出现透亮区,其周围可见低密度的软组织阴影,诱导膜中有很多毛细血管内皮细胞,排列无序,诱导膜组织中IL-6、TNF-α水平显著降低,VEGF、TGF-β1和BMP-2蛋白表达显著升高(均P<0.05);与 PMMA骨水泥组比较,PMMA骨水泥+木犀草素组骨缺损区的骨水泥形态不规则程度和周围的软组织阴影范围更为显著,诱导膜中有大量毛细血管内皮细胞,排列整齐,诱导膜组织中IL-6、TNF-α水平显著降低,VEGF、TGF-β1和BMP-2蛋白表达显著升高(均P<0.05).植骨后第7周,三组骨缺损区均可见染色较均匀的新生骨组织,新生骨中含有成骨细胞及骨基质;与对照组比较,PMMA骨水泥组骨缺损区出现明显的骨性桥接,骨愈合迹象得到显著改善,Lane-Sandhu X射线评分、新生骨占比及成骨细胞数显著升高(均P<0.05);与PMMA骨水泥组比较,PMMA骨水泥+木犀草素组骨缺损区的骨性桥接更为连续和成熟,骨缺损间隙显著缩小,骨愈合进程更为迅速和完全,Lane-Sandhu X射线评分、新生骨占比及成骨细胞数显著升高(均P<0.05).结论:载木犀草素的PMMA骨水泥能通过抑制炎症与增强诱导膜成骨活性协同促进桡骨缺损大鼠的新骨形成与骨愈合.
Objective:To explore the effect of polymethyl methacrylate(PMMA)bone cement loaded with lute-olin on osteogenic repair in rats with radial defects.Methods:The rat model of radius defect was constructed and di-vided into control group,PMMA bone cement group and PMMA bone cement+luteolin group,with 18 rats in each group.No material was implanted in the bone defect of the control group;PMMA bone cement group rats were im-planted with PMMA bone cement without loaded luteolin at the bone defect site;PMMA bone cement+luteolin group was implanted with PMMA bone cement loaded with luteolin.At the fifth week after modeling,autologous cancellous bone was implanted at the incision of the original radius in 6 rats from each group.The general state of the rats was observed during the experiment;X-ray was used to observe the contour of bone cement,the reaction range of bone cement and surrounding tissue,and bone healing in the bone defect area;hematoxylin-eosin staining was used to observe the morphological changes of induces membrane and newly formed bone in bone defect areas,and the propor-tion of new bone and the number of osteoblasts were evaluated;the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in induces membrane tissue were detected by enzyme-linked immunosorbent assay;the expressions of vascular endothelial growth factor(VEGF),transforming growth faction-β1(TGF-β1)and bone morphogenetic protein-2(BMP-2)in induces membrane tissue were detected by western blotting.Results:During the experiment,all animals had good appetite,physical activity,and no deaths;One week after surgery,edema,congestion,swelling,and pain were observed in all surgical sites in each group.At the 5th week after modeling,compared with control group,the bone cement contour in the bone defect area of the PMMA bone cement group became irregular,and the interface between the bone cement and the surrounding tissue appeared transparent area,and the low-density soft tissue shad-ow could be seen around it,many capillary endothelial cells in the induced membrane were disordered,and the levels of IL-6 and TNF-α in the induces membrane tissue were significantly reduced,the protein expressions of VEGF,TGF-β1 and BMP-2 were significantly increased(all P<0.05).Compared with PMMA bone cement group,the bone cement shape irregularity and soft tissue shadow around the bone defect area in the PMMA bone cement+luteolin group were more significant,and the induced membranes contained a large number of capillary endothelial cells ar-ranged neatly,and the levels of IL-6 and TNF-α in the induces membrane tissue were significantly reduced,the pro-tein expressions of VEGF,TGF-β1 and BMP-2 were significantly increased(all P<0.05).At the 7th week after bone grafting,uniformly stained new bone tissue was found in the bone defect area of the 3 groups,and the new bone con-tained osteoblasts and bone matrix;compared with control group,the PMMA bone cement group showed obvious bone bridging in the bone defect area,and the signs of bone healing were significantly improved,Lane-Sandhu X-ray score,the proportion of new bone and osteoblast number were significantly increased(all P<0.05).Compared with PMMA bone cement group,the bone bridge in the bone defect area of the PMMA bone cement+luteolin group was more continuous and mature,the bone defect gap was significantly reduced,the bone healing process was more rapid and complete,and the Lan-Sandhu X-ray score,the proportion of new bone and osteoblast number were significantly increased(all P<0.05).Conclusion:PMMA bone cement loaded with luteolin can synergistically promote new bone formation and bone healing by in-hibiting inflammation and enhancing the osteogenic activity of induced membrane in rats with radius defects.
