脊髓型颈椎病术后病人睡眠质量发展轨迹及其影响因素OA
Sleep quality trajectories and its influencing factors in postoperative patients with cervical spondylotic myelopathy
目的:探讨脊髓型颈椎病术后病人睡眠质量的发展轨迹,并分析其影响因素.方法:采用便利抽样法,选取2023年8月—2024年8月在我院接受脊髓型颈椎病手术治疗的244例病人为研究对象.采用中文版匹兹堡睡眠质量指数量表(PSQI)评估病人基线、术后1周、术后1个月、术后3个月的睡眠质量,通过潜变量增长混合模型分析睡眠质量发展轨迹分类,通过单因素分析及Logistic回归分析探讨发展轨迹的独立影响因素.结果:244例脊髓型颈椎病术后病人睡眠质量可分为 3个类别,即高质量-改善型 65例(26.6%)、低质量-改善型109例(44.7%)、低质量-降低型70例(28.7%).Logistic回归分析结果显示,年龄、家庭人均月收入、术前病程、病变涉及节段、术前中文版日本骨科学会评分(JOA)、术前视觉模拟评分法(VAS)评分、术前抑郁自评量表(SDS)评分、术前焦虑自评量表(SAS)评分均是脊髓型颈椎病术后病人睡眠质量发展轨迹的影响因素(P<0.05).结论:脊髓型颈椎病术后病人的睡眠质量发展轨迹存在一定异质性,且受多种因素影响,提示医护人员可根据其影响因素预测病人术后睡眠质量的发展轨迹,并制定个性化的护理方案以提高病人睡眠质量.
Objective:To explore the dynamic sleep quality trajectories and analyze the influencing factors in postoperative patients with cervical spondylotic myelopathy.Methods:A total of 244 postoperative patients with cervical spondylotic myelopathy in our hospital were selected as the study subjects by convenience sampling from August 2023 to August 2024.The Chinese version of the Pittsburgh Sleep Quality Index(PSQI)was used to assess sleep quality of patients at baseline,1 week,1 month,and 3 months after operation.Latent class growth modeling was employed to analyze the classification of sleep quality trajectories.Univariate analysis and Logistic regression analysis were used to explore the influencing factors of the trajectories.Results:The sleep quality trajectories of the 244 postoperative patients with cervical spondylotic myelopathy could be categorized into 3 categories,namely high quality-improving type(65 cases,26.6%),low quality-improving type(109 cases,44.7%),and low quality-declining type(70 cases,28.7%).Logistic regression analysis showed that age,monthly per-capita household income,preoperative disease duration,number of involved spinal segments,preoperative Chinese version of Japanese Orthopaedic Association(JOA)score,preoperative Visual Analogue Scale(VAS)score,preoperative Self-Rating Depression Scale(SDS)score,and preoperative Self-Rating Anxiety Scale(SAS)score were all influencing factors for the sleep quality trajectories in postoperative patients wtih cervical spondylotic myelopathy(P<0.05).Conclusions:There is certain heterogeneity in the sleep quality trajectories in postoperative patients with cervical spondylotic myelopathy,which are influenced by multiple factors.It suggests that healthcare professionals can predict postoperative sleep quality trajectories of patients based on these influencing factors and develop personalized nursing plans to improve patient's sleep quality.
