目的 探讨成年人中不同肥胖代谢表型与高尿酸血症(hyperuricemia,HUA)风险之间的相关性.方法 采用横断面研究,提取2011年至2018年美国国家健康与营养调查数据库中年龄≥20岁且具有完整体质指数、腰围、血尿酸、空腹血糖、高密度脂蛋白胆固醇、甘油三酯、收缩压、舒张压数据的成年人样本(n=7 394).根据肥胖和代谢异常状况,将样本分为四组:代谢健康非肥胖(metabolically healthy non-obese,MHNO)、代谢不健康非肥胖(metabolically unhealthy non-obese,MUNO)、代谢健康肥胖(metabolically healthy obesity,MHO)和代谢不健康肥胖(metabolically unhealthy obesity,MUO).采用加权多变量Logistic回归模型分析肥胖代谢表型与HUA的关联,并使用限制性立方样条评估各肥胖、代谢指标与HUA风险的剂量反应关系;通过亚组分析和交互检验评估结果的稳健性.结果 该研究人群HUA患病率为22.7%.以MHNO组为参照,经过多变量校正后,MUNO、MHO、MUO组HUA风险依次升高,比值比(OR)和95%置信区间(95%CI)分别为2.12(1.50~3.00)、2.16(1.69~2.76)、4.53(3.41~6.02),P值均<0.001.限制性立方样条分析显示各肥胖、代谢指标均与HUA风险呈非线性剂量反应关系.亚组分析发现,肥胖代谢表型与HUA的关联在性别、种族、教育程度和饮酒状况上存在显著交互作用(P交互<0.05).该关联在男性和青年(20~39岁)人群中尤为显著.结论 MUO个体具有的HUA风险最高,提示临床评估HUA风险需超越传统BMI分类,应整合血压、血糖、血脂等代谢指标进行综合分型,以实现高危人群的精准识别、分层管理与早期干预.

Objective The aim of this study was to explore the correlation between different obesity metabolic phenotypes and the risk of hyperuricemia(HUA)in adults.Methods A cross-sectional study was conducted using data from the 2011-2018 National Health and Nutrition Examination Survey(NHANES)database,extracting adult samples(n=7394)aged≥20 years with complete measurements of body mass index,waist circumference,serum uric acid,fasting blood glucose,high-density lipoprotein cholesterol,triglycerides,systolic blood pressure,and diastolic blood pressure.The sample was categorised into four groups based on obesity and metabolic status:metabolically healthy non-obese(MHNO),metabolically unhealthy non-obese(MUNO),metabolically healthy obe-sity(MHO),and metabolically unhealthy obesity(MUO).We employed weighted multivariate logistic regression models to analyse associations between obesity-metabolic phenotypes and HUA,utilising restricted cubic splines to assess dose-response relationships between obesity and metabolic indicators and HUA risk.Robustness of findings was evaluated through subgroup analyses and interac-tion tests.Results The prevalence of HUA in the study cohort was 22.7%.Using the MHNO group as reference,after multivariate adjustment,the risk of HUA progressively increased in the MUNO,MHO,and MUO groups,with odds ratios(OR)and 95%confi-dence intervals(95%CI)being 2.12(1.50~3.00),2.16(1.69~2.76),and 4.53(3.41~6.02),respectively,all with P values<0.001.Restricted cubic spline analysis revealed a non-linear dose-response relationship between HUA risk and each obesity/metabolic indicator.Subgroup analyses revealed significant interactions between obesity-metabolic phenotypes and HUA risk across gender,eth-nicity,educational attainment,and alcohol consumption status(P for interaction<0.05).This association was particularly pronounced among males and younger individuals(aged 20~39 years).Conclusion Individuals with the MUO phenotype carry the highest risk of HUA,which suggests that clinical assessment of HUA risk should extend beyond traditional BMI categories to include a comprehensive integration of metabolic indicators such as blood pressure,blood glucose,and lipids.This approach enables precision identification,stratified management,and early intervention for high-risk populations.
