Comparative analysis of efficacy between transscleral cyclophotocoagulation and ahmed glaucoma valve im-plantation in the treatment of advanced refractory glaucoma

目的 探讨经巩膜睫状体光凝术(TSCPC)与Ahmed引流阀植入术在晚期难治性青光眼治疗中的临床疗效、安全性和经济性.方法 回顾性病例分析.2014年2~11月在江苏省人民医院眼科诊治的70例(72只眼)晚期难治性青光眼患者,其中36只眼行TSCPC,36只眼行Ahmed引流阀植入术.记录术前及术后各随访时间点的眼压、术前和术后6个月眼部疼痛情况、最佳矫正视力、使用的降眼压药物种类及并发症,并分析手术的完全成功率和条件成功率(使用或不使用降眼压药物情况下6 mmHg≤眼压≤21 mmHg).结果 两组患者术后的眼压及降眼压药物种类较术前显著下降(P<0.05).TSCPC组眼压由(42.3±9.9)mmHg降低至(16.4±8.0)mmHg,使用的药物种类由(2.6±0.6)种减少为(0.4±0.7)种;Ahmed组眼压由(40.3±7.6)mmHg降低至(23.0±12.4)mmHg,使用的药物种类由(2.4±0.7)种减少为(0.7±0.9)种.术后6个月的条件成功率TSCPC组为72.2%,Ahmed组为66.7%;术后6个月的完全成功率光凝组为63.9%,Ahmed组为44.4%(P>0.05).TSCPC组有2只眼、Ahmed阀组有4只眼发生了并发症.结论 TSCPC与Ahmed引流阀植入术均能够安全有效地降低晚期难治性青光眼患者眼压并维持视力.TSCPC更简单、安全、经济,能更有效控制眼压和缓解患者眼痛,减少用药种类.

Objective To assess the clinical efficacy,safety,and cost-effectiveness of transscleral cyclophotoco-agulation(TSCPC)versus ahmed glaucoma valve implantation in patients with advanced refractory glaucoma.Methods Retrospective case analysis.From February 2014 to November 70 patients(72 eyes)with advanced refractory glaucoma were diagnosed and treated in the Ophthalmology Department of Jiangsu Provincial People's Hospital,of whom 36 eyes un-derwent transscleral cyclophotocoagulation(TSCPC)and 36 eyes received ahmed glaucoma valve implantation.The intraocu-lar pressure(IOP)at each follow-up time point before and after the surgery,the ocular pain before and at 6 months after the surgery,the best corrected visual acuity(BCVA),number of anti-glaucoma medications and complications were recorded.Complete success rate of the surgery and conditional success rate of the surgery(6 mmHg≤IOP≤21 mmHg with or without medications)were analyzed.Results IOP and the number of anti-glaucoma medications decreased significantly after the surgery(P<0.05).The IOP in the TSCPC group decreased from(42.3±9.9)mmHg(medications:2.6±0.6 kinds)to(16.4±8.0)mmHg(0.4±0.7),and the IOP in the ahmed valve group decreased from(40.3±7.6)mmHg(2.4±0.7)to(23.0±12.4)mmHg(0.7±0.9).The conditional success rate was 72.2%in the TSCPC group and 66.7%in the ahmed valve group at 6 months postoperatively.The complete success rate was 63.9%in the photocoagulation group and 44.4%in the ahmed valve group(P>0.05).Complications occurred in 2 eyes in the photocoagulation group and 4 eyes in the ahmed valve group.Conclusions Both transscleral cyclophotocoagulation and ahmed glaucoma valve implantation can safely and effectively reduce the IOP and maintain the visual acuity of patients with advanced refractory glaucoma.Transscleral cyclophotocoagu-lation is simpler,safer and more economical.It can more effectively control the IOP,significantly relieve ocular pain,and reduce the variety of medications.
