Study on the defocus status of peripheral retina in myopic patients

目的 探究近视患者视网膜周边离焦状态,等效球镜度(SER)、眼轴(AL)与视网膜周边离焦状态的相关性及佩戴定制离焦镜片1年后眼轴增长快慢患者戴镜前视网膜离焦状态是否有差异.方法 回顾性病例研究.通过查阅医院电子病案系统收集2023年1~9月在我院验配定制离焦镜片患者51例,提取包括年龄、性别、屈光度、镜片类型等相关信息,所有受检者均取右眼数据进行统计分析(共51只眼),根据眼轴的变化情况分为增长快组(年增长≥0.2 mm)37例(37只眼),增长慢组(年增长<0.2 mm)14例(14只眼),主要指标包括:AL测量;多光谱屈光地形图(MRT)测量视网膜离焦量,正值为远视性离焦,负值为近视性离焦.以黄斑中心凹为圆心,视网膜总离焦量(TRDV),视网膜15°范围离焦量记录为RDV-15°、视网膜30°范围离焦量记录为RDV-30°、视网膜45°范围离焦量记录为RDV-45°、视网膜上方离焦量记录为RDV-S、视网膜下方离焦量记录为RDV-I、视网膜颞侧离焦量记录为RDV-T、视网膜鼻侧离焦量记录为RDV-N.增长快慢组不同区域离焦量差异.结果 (1)RDV-15°、RDV-30°呈近视性离焦,RDV-45°呈远视性离焦,RDV-S呈近视性离焦、RDV-I、RDV-T、RDV-N呈现远视性离焦.(2)TRDV、RDV-30°、RDV-45°、RDV-S、RDV-T、RDV-N与SER呈负相关,与AL呈正相关(P<0.05),RDV-15°、RDV-I与SER、AL均无关(P>0.05).(3)两组患者视网膜周边离焦状态差异无统计学意义(P>0.05).结论 (1)近视患者视网膜周边相对离焦随距黄斑中心距离的增加而增加,且视网膜离焦具有不对称,RDV-15°、RDV-30°呈近视性离焦,RDV-45°呈远视性离焦,RDV-S呈近视性离焦、RDV-I、RDV-T、RDV-N呈现远视性离焦.(2)TRDV、RDV-30°、RDV-45°、RDV-S、RDV-T、RDV-N与SER呈负相关,与AL呈正相关,RDV-15°、RDV-I与SER、AL均无关.(3)定制离焦框架眼镜戴镜前的视网膜离焦状态可能与眼轴进展快无相关性.

Objective To investigate the peripheral defocus status of myopic patients,the correlation between equivalent spherical power,axial length,and peripheral defocus status of the retina,and whether there is a difference in the rate of axial growth of the retina before wearing customized defocus lenses for one year.Methods Retrospective study.By consulting the hospital's electronic medical record system,51 patients who underwent customized defocusing lens fit-ting in our hospital from January 2023 to September were collected.Relevant information including age,gender,refractive power,lens type,etc.was extracted.All subjects'right eye data were collected for statistical analysis(a total of 51 eyes).According to the changes in eye axis,they were divided into a fast growth group(annual growth≥0.2 mm)of 37 cases(a to-tal of 37 eyes)and a slow growth group(annual growth<0.2 mm)of 14 cases(a total of 14 eyes),with the main indicators including eye axis length;Multispectral Refractive Topography(MRT)measures retinal defocus,with positive values indi-cating farsightedness and negative values indicating nearsightedness.Taking the macular fovea as the center,the total defo-cus amount of the retina is recorded as TRDV,the defocus amount within 15° of the retina is recorded as RDV-15°,the de-focus amount within 30 ° of the retina is recorded as RDV-30°,the defocus amount within 45° of the retina is recorded as RDV-45°,the defocus amount above the retina is recorded as RDV-S,the defocus amount below the retina is recorded as RDV-I,the defocus amount on the temporal side of the retina is recorded as RDV-T,and the defocus amount on the nasal side of the retina is recorded as RDV-N.Differences in defocus amount in different regions of the growth rate group.Results(1)RDV-15°and RDV-30°show myopic defocus,RDV-45°shows hyperopic defocus,RDV-S shows myopic defo-cus,RDV-I,RDV-T,and RDV-N show hyperopic defocus.(2)TRDV,RDV-30°,RDV-45°,RDV-S,RDV-T,and RDV-N are negatively correlated with SER and positively correlated with AL(all P<0.05),while RDV-15°and RDV-I are not correlated with SER or AL(P>0.05).(3)There was no statistically significant difference in the defocus status of the retina between the two groups of patients(P>0.05).Conclusions(1)The relative defocus around the retina of myopic patients increases with the increase of distance from the macular center,and the retinal defocus is asymmetric.RDV-15°and RDV-30°show myopic defocus,RDV-45°shows hyperopic defocus,RDV-S shows myopic defocus,and RDV-I,RDV-T,and RDV-N show hyperopic defocus.(2)TRDV,RDV-30,RDV-45°,RDV-S,RDV-T,and RDV-N are negatively correlated with SER and positively correlated with AL,while RDV-15°and RDV-I are not related to SER or AL.(3)The defocused state of the retina before wearing customized defocused frame glasses may not be correlated with rapid axial progression.
