目的 测量单眼视网膜静脉阻塞(RVO)对侧眼的视网膜神经纤维层(RNFL)厚度和视盘旁放射状毛细血管丛(RPCP)层毛细血管灌注密度,并探究RVO对侧眼中上述两参数的相关性,解释RVO对侧眼"非健康眼"这一现象,为揭示RVO致病机理提供参考.方法 回顾性病例对照研究.选取了2020年6月1日至12月31日就诊于内蒙古医科大学附属医院眼科的50名单眼RVO患者的对侧眼纳入实验组,将同时期50只健康眼纳入对照组.运用相干光层析成像术(OCT)和相干光层析血管成像术(OCTA)技术分别采集所有受试者视盘旁RNFL厚度图和RPCP层血流图,利用Matlab软件计算毛细血管灌注密度.结果 实验组与对照组盘周颞侧区(T区)的平均RNFL厚度差异有统计学意义(P<0.05);实验组与对照组视盘旁的平均毛细血管灌注密度差异有统计学意义(P<0.05);鼻上区(NS区)、鼻侧区(N区)、全周RNFL厚度与视盘旁毛细血管灌注密度呈显著正相关(r=0.432,P=0.002;r=0.342,P=0.015;r=0.325,P=0.021);视网膜中央静脉阻塞(CRVO)与视网膜分支静脉阻塞(BRVO)亚组间的视盘旁毛细血管灌注密度与对应各区RNFL厚度差异均无统计学意义.结论 RVO对侧眼的视盘旁RPCP层的毛细血管灌注密度较正常眼显著减小,盘周T区的RNFL厚度较正常眼显著变薄.此外,RVO对侧眼的视盘旁毛细血管灌注密度与NS区、N区和全周RNFL厚度呈显著正相关.CRVO和BRVO的视盘旁毛细血管密度和RNFL厚度无差异.看似正常的对侧眼并不是"健康眼",其盘周的视网膜结构和微循环已发生改变.

Objective To measure the retinal nerve fiber layer(RNFL)thickness and radial peripapillary capil-lary plexus(RPCP)perfusion density(PD)in the fellow eyes of unilateral RVO,explore the correlation between these two parameters in the fellow eyes,explain the phenomenon that unilateral RVO have"unhealthy fellow eyes",and provide a reference for revealing the pathogenesis of RVO.Methods Retrospective case-control study.The fellow eyes of 50 pa-tients with unilateral RVO who were admitted to the Department of Ophthalmology,the Affiliated Hospital of Inner Mongo-lia Medical University from June 1 to December 31,2020 were enrolled in the experimental group,and 50 healthy eyes during the same period were included in the control group.Optical coherence tomography(OCT)and optical coherence to-mography angiography(OCTA)were used to acquire peripapillary RNFL thickness maps and RPCP blood flow maps of all subjects,respectively.Matlab software was employed to calculate the capillary perfusion density.Results There was a sta-tistically significant difference in the average RNFL thickness of the peripapillary temporal region(T region)between the experimental group and the control group(P<0.05).The difference in the average peripapillary capillary perfusion density between the two groups was also statistically significant(P<0.05).The RNFL thickness in the superonasal region(NS re-gion),nasal region(N region)and the entire circumference was significantly positively correlated with peripapillary capil-lary perfusion density(r=0.432,P=0.002;r=0.342,P=0.015;r=0.325,P=0.021).No statistically significant differences were observed in peripapillary capillary perfusion density and corresponding regional RNFL thickness between the central retinal vein occlusion(CRVO)subgroup and the branch retinal vein occlusion(BRVO)subgroup.Conclusions The cap-illary perfusion density of the peripapillary RPCP in the fellow eyes of RVO patients is significantly lower than that in healthy eyes,and the RNFL thickness in the peripapillary T region is significantly thinner than that in normal eyes.In ad-dition,the peripapillary capillary perfusion density of the fellow eyes of RVO patients is significantly positively correlated with the RNFL thickness in the NS region,N region and the entire circumference.There are no differences in peripapillary capillary perfusion density and RNFL thickness between CRVO and BRVO patients.The seemingly normal fellow eyes are not actually"healthy eyes",as their peripapillary retinal structure and microcirculation have already undergone changes.
