目的 比较Pentacam与IOL Master测量的角膜屈光力值在角膜形态正常的白内障患者人工晶状体屈光力计算中的准确性.方法 前瞻性临床研究.纳入38例(52只眼)拟行白内障超声乳化吸除联合人工晶状体植入术的角膜形态正常的白内障患者.术前分别采用Pentacam和IOL Master检查.采集的数据包括:由Pentacam测量的Sim K、角膜中央1.0~7.0 mm直径EKR(Holladay equivalent K reading)、TNP(True net power)和由IOL Master测量的Km,将不同的K值分别联合4种IOL计算公式(SRK/T、Hoffer Q、Haigis和Holladay I)计算每种K值的IOL度数.测量患者术后3个月术眼验光结果,根据验光结果计算各种K值的屈光预测误差(ME)和平均绝对预测误差(MAE).采用重复测量方差分析比较两种仪器所测K值联合各计算公式计算IOL屈光预测误差之间的差异.结果 两种仪器所测不同K值联合四种IOL计算公式的MAE比较,IOL Master测量所得Km值MAE最小,分别为:SRK/T公式:(0.61±0.51)D、Hoffer Q公式(0.59±0.44)D、Haigis公式:(0.80±0.86)D及Holladay I公式(0.6±0.46)D,Pentacam测量所得Sim K值MAE次之,7.0 mm EKR值MAE最大.在1.0~7.0 mm EKR值的MAE比较,3.0 mm EKR值MAE最小.结论 在角膜形态正常白内障患者中,应用Pentacam测量所得K值相对IOL Master测量所得Km值不能提高IOL屈光力计算的准确性.

Objective To evaluate keratometry(K)readings obtained with Pentacam and intraocular lenses(IOL)Master in eyes scheduled for cataract surgery,and to compare the predicted and actual refractive outcomes.Methods Pro-spective study.52 eyes of 38 cataract patients having phacoemulsification and in-the-bag IOL implantation were studied.Before the operation,Pentacam and IOL Master were used to measure corneal refractive power,respectively.And the col-lected data included:Sim K,equivalent K reading(EKR)at 1.0 to 7.0 mm of corneal diameters,True net power(TNP)were measured by Pentacam system and Km measured by IOL master.The IOL power was calculated using 4 formulas.Sta-tistical analysis of refractive outcomes was performed.Results The Km gave the lowest mean absolute error(MAE)with all formulas.The second lowest MAE was the Sim K by the Pentacam.The highest MAE was the Scheimpflug equivalent K at 7.0 mm.Among the equivalent K at 1.0~7.0 mm corneal diameters,equivalent K at 3.0 mm gave the lowest MAE.Con-clusions In cataract patients with normal corneal morphology,the K value measured by Pentacam can not improve the ac-curacy of IOL refractive power calculation compared with Km value measured by IOL Master.
