目的:比较机器人辅助与全胸腔镜辅助下心脏肿瘤切除术的临床效果,评估机器人手术系统的安全性与有效性,并总结初步应用经验.方法:回顾性选取 2018 年 9 月—2025 年 3 月于南京医科大学第一附属医院心脏大血管外科接受微创心脏肿瘤切除术的 59 例患者为研究对象.根据手术方式,将患者分为机器人组(n=24)与胸腔镜组(n=35).比较两组患者的围手术期指标及随访结果.结果:患者均成功完成微创手术.与胸腔镜组相比,机器人组患者术后 24 h引流量更少,体外循环时间、主动脉阻断时间及术后呼吸机使用时间均更短,差异有统计学意义(P<0.05);两组术后住院天数、手术时间、术中红细胞输注量、术后ICU停留时间、动静脉插管型号、术后发生脑梗及同期行其他手术情况等方面比较,差异均无统计学意义(P>0.05).随访期间,患者均未出现肿瘤复发.结论:机器人辅助心脏肿瘤切除术安全、有效,与全胸腔镜手术均能获得良好的随访结果.但对于瘤蒂宽大或肿瘤体积较大的患者,机器人辅助手术更具优势.

Objective:To compare the clinical outcomes of robot-assisted and totally thoracoscopic cardiac tumor resection,evaluate the safety and efficacy of the robotic surgical system and summarize preliminary experience.Methods:A retrospective analysis was conducted on 59 patients who underwent minimally invasive cardiac tumor resection at the Department of Cardiovascular Surgery at the First Affiliated Hospital of Nanjing Medical University between September 2018 and March 2025.Based on the surgical approach,patients were divided into the robotic group(n=24)and the thoracoscopic group(n=35).Perioperative indicators and follow-up outcomes were compared between the two groups.Results:All surgeries were successfully completed.Compared to the thoracoscopic group,the robotic group showed significantly lower 24h drainage,shorter cardiopulmonary bypass time,aortic cross-clamp time,and postoperative ventilator-assisted time(P<0.05).There were no statistically significant differences between the two groups in terms of postoperative hospital stay,total operative time,intraoperative RBC infusion,postoperative length of ICU stay,size of arteriovenous catheter,postoperative incidence of stroke,and co-occurrence of other surgeries(P>0.05).No tumor recurrence was observed in any patient during the follow-up.Conclusion:Robot-assisted cardiac tumor resection is safe and effective,which can achieve comparable long-term outcomes to total thoracoscopic surgery.For patients with broad-based or large-volume tumors,robot-assisted surgery has significant advantages.
