舒筋活络洗剂熏洗联合体外冲击波疗法治疗跟痛症气滞血瘀证30例临床观察OA
Clinical observation on 30 cases of painful heel syndrome with qi stagnation and blood stasis treated by Shujin Huoluo Decoction(舒筋活络洗剂)fumigation and washing combined with extracorporeal shock wave therapy
目的 观察舒筋活络洗剂熏洗联合体外冲击波疗法治疗跟痛症气滞血瘀证的临床疗效.方法 将60例跟痛症气滞血瘀证患者随机分为对照组和治疗组,各30例.对照组采用体外冲击波疗法治疗,治疗组采用舒筋活络洗剂熏洗联合体外冲击波疗法治疗,治疗15 d,并随访1个月.比较2组的临床疗效及治疗前后晨起行走时疼痛视觉模拟评分法(VAS)评分、Maryland足功能评分,并评价2组的复发情况及安全性.结果 治疗后2组晨起行走时疼痛VAS评分明显降低,Maryland足功能评分明显升高,与同组治疗前比较差异均有统计学意义(P＜0.01),且治疗组改善更明显,与对照组治疗后比较差异均有统计学意义(P＜0.05或P＜0.01);对照组总有效率为80.00％(24/30),治疗组为96.67％(29/30),2组比较差异有统计学意义(P＜0.05);随访1个月后,对照组有3例患者复发,治疗组未观察到复发病例;治疗期间,治疗组未出现皮肤红肿或过敏等不良反应,对照组有6例患者出现疼痛不耐受及局部肿胀等不良反应.结论 舒筋活络洗剂熏洗联合体外冲击波疗法治疗跟痛症气滞血瘀证疗效显著,可有效缓解患者晨起行走时疼痛症状,显著改善足功能,其近期及远期疗效均明显优于单纯体外冲击波疗法,且安全性较高,值得临床推广应用.
Objective To observe the clinical efficacy of the combination of Shujin Huoluo Decoction(舒筋活络洗剂)fumigation and washing and extracorporeal shock wave therapy(ESWT)in the treatment of qi stagnation and blood stasis syndrome of painful heel syndrome.Methods 60 patients with qi stagnation and blood stasis syn-drome of painful heel syndrome were randomly divided into a control group and a treatment group,with 30 cases in each group.The control group was treated with ESWT,while the treatment group was treated with Shujin Huoluo Decoction fumigation and washing combined with ESWT.The treatment lasted for 15 days,and a one-month follow-up was conducted.The clinical efficacy,visual analogue scale(VAS)scores for pain during morning walking,and Maryland foot function scores before and after treatment were compared between the two groups.The recurrence and safety of the two groups were also evaluated.Results After treatment,the VAS scores for pain during morning walking in both groups were significantly reduced,and the Maryland foot function scores were significantly increased.The differences compared with the same group before treatment were statistically significant(P＜0.01).Moreover,the improvement in the treatment group was more obvious,and the differences compared with the control group after treatment were statistically significant(P＜0.05 or P＜0.01).The total effective rate of the control group was 80.00％(24/30),and that of the treatment group was 96.67％(29/30).The difference between the two groups was statistically significant(P＜0.05).After one month of follow-up,3 patients in the control group had a recur-rence,while no recurrence was observed in the treatment group.During the treatment period,no adverse reactions such as skin redness,swelling,or allergy occurred in the treatment group,while 6 patients in the control group expe-rienced adverse reactions such as pain intolerance and local swelling.Conclusion The combination of Shujin Huoluo Decoction fumigation and washing with ESWT is effective in treating painful heel syndrome with qi stagna-tion and blood stasis.It can effectively relieve the pain symptoms during morning walking and significantly improve foot function.Its short-term and long-term efficacy are significantly better than those of ESWT alone,and it has high safety,making it worthy of clinical promotion and application.
