目的 探讨首乌丸与二仙汤对卵巢早衰(POF)模型大鼠性激素、细胞凋亡、卵巢颗粒细胞(GCs)内线粒体形态及线粒体自噬相关因子同源性磷酸酶张力蛋白诱导激酶1(PINK1)、E3泛素连接酶(Parkin)的作用差异.方法 将25只雌性SD大鼠采用随机数字表法分为空白组、模型组、二仙汤组、首乌丸预防组及补佳乐组,每组5只.除空白组外,其余5组腹腔注射去氧乙烯基环己烯溶液建立POF大鼠模型,空白组、模型组予生理盐水灌胃,其余组予相应药物灌胃干预.检测各组大鼠血清性激素[促性腺激素释放激素(GnRH)、促卵泡生成素(FSH)、促黄体生成素(LH)、抗缪勒氏管激素(AMH)、雌二醇(E2)]水平,卵巢组织PINK1、Parkin基因及蛋白表达水平,观察卵巢组织形态学、卵巢GCs超微结构及卵巢细胞凋亡情况.结果 与模型组比较,首乌丸预防组、二仙汤组及补佳乐组血清LH、FSH、GnRH水平,卵巢组织PINK1 mRNA表达水平及Parkin mRNA与蛋白表达水平,卵巢细胞凋亡指数均明显降低(P<0.01),血清AMH、E2水平均明显升高(P<0.01),首乌丸预防组及补佳乐组PINK1蛋白表达水平均明显降低(P<0.01);与首乌丸预防组比较,二仙汤组血清GnRH、AMH水平均明显降低(P<0.05),卵巢组织Parkin mRNA,PINK 1、Parkin蛋白表达水平,卵巢细胞凋亡指数均明显升高(P<0.05).与空白组比较,模型组大鼠卵巢萎缩,GCs层次减少,各级卵泡数量亦较少;卵巢GCs超微结构严重受损,胞质内线粒体明显减少,线粒体形态异常.与模型组比较,首乌丸预防组与二仙汤组大鼠卵巢萎缩均有不同程度减轻,各级卵泡及黄体数量均增多,凋亡细胞显著减少;首乌丸预防组卵巢GCs超微结构明显改善,线粒体等细胞器结构及形态均明显改善,线粒体数量增加且形态接近正常;二仙汤组电镜下GCs超微结构亦明显改善,且线粒体数量最多,不同放大倍数下GCs内几乎所有线粒体均呈现肿胀形态.结论 首乌丸对POF模型大鼠血清AMH、线粒体自噬因子PINK1、Parkin及细胞凋亡的调节作用优于二仙汤,二仙汤对血清GnRH的调节作用优于首乌丸.据此推测二仙汤主要促进线粒体氧化供能,类似于"阳化气"作用;而首乌丸主要增加线粒体能量储备,类似于"阴成形"作用.

Objective To compare the differential effects of Shouwu Wan(首乌丸)and Erxian Tang(二仙汤)on sex hormones,cell apoptosis,mitochondrial morphology and autophagy-related factors(PINKl/Parkin)in ovarian granulosa cells(GCs)of premature ovarian failure(POF)model rats,and explore their mechanistic implications.Methods 25 female SD rats were randomly divided into 5 groups(n=5):blank group,model group,Erxian Tang(EXT)group,Shouwu Wan prevention(SWW)group,and Progynova(estradiol valerate)group.Except for the blank group,POF models were established via intraperitoneal injection of 4-vinylcyclohexene diepoxide(VCD).The blank and model groups received saline gavage,while other groups were administered corresponding drugs.Serum sex hor-mone levels[gonadotropin-releasing hormone(GnRH),follicle-stimulating hormone(FSH),luteinizing hormone(LH),anti-Müllerian hormone(AMH),and estradiol(E2)],ovarian PINK1 and Parkin mRNA/protein expression,ovarian histomorphology,GCs ultrastructure,and ovarian cell apoptosis were analyzed.Results Compared with the model group,the serum levels of LH,FSH,and GnRH,the mRNA expression level of PINK1 and the mRNA/protein expres-sion levels of Parkin in ovarian tissue,the ovarian cell apoptosis index in the SWW group,EXT group and Progynova group,as well as the protein expression level of PINK1 in the SWW group and the Progynova group were all signifi-cantly decreased(P<0.01),while the serum levels of AMH and E2 were significantly increased(P<0.01).Compared with the SWW group,the EXT group showed lower serum GnRH and AMH levels(P<0.05),higher Parkin mRNA,PINK1,and Parkin protein expression levels,and increased apoptosis indices(P<0.05).Histologically,the model group displayed ovarian atrophy,reduced GCs layers,and fewer follicles.Ultrastructural analysis revealed severe GCs damage,mitochondrial depletion,and abnormal mitochondrial morphology.SWW and EXT groups showed mitigated ovarian atrophy,increased follicles and corpora lutea,reduced apoptosis,and improved GCs ultrastructure.SWW grou-prestored mitochondrial structure and quantity,while EXT group induced mitochondrial swelling despite higher mito-chondrial numbers.Conclusion Shouwu Wan demonstrated superior regulation of AMH,mitophagy factors(PINK1/Par-kin),and apoptosis inhibition,whereas Erxian Tang better modulated GnRH.This suggests Erxian Tang enhances mito-chondrial oxidative energy production,which is similar to the TCM theory of"yang transforming qi",while Shouwu Wan promotes mitochondrial energy storage,analogous to"yin shaping form".
