神经外科发展现已迈入以人工智能(artificial intelligence,AI)为核心驱动力的智能神经外科新阶段.智能神经外科是以现代神经医学为基础,融合AI、机器学习、手术机器人、脑机接口、大数据、数字孪生及神经调控等前沿技术,通过人机协同模式构建覆盖"术前精准评估-术中实时导航与辅助-术后精细化管理"全流程的智能化诊疗体系.其核心技术支柱包括深度学习驱动的影像自动分割与分子分型预测、手术机器人实现亚毫米级精准操作、脑机接口建立脑与外设直接通信通路、多模态数据融合整合临床、基因组与影像信息,构建预测模型优化决策、数字孪生与神经调控技术实现个体化治疗模拟与可逆性神经调节.临床应用已广泛覆盖脑肿瘤的最大化安全切除、帕金森病与癫痫等功能性疾病的参数个性化调控、脑血管病的微创介入治疗,并在军事医学领域展现出颅脑战创伤快速智能救治的重大价值.然而,数据隐私、算法责任、伦理规范及技术普及不均等问题仍是其发展面临的严峻挑战.智能神经外科的本质是现代前沿技术赋能传统学科,形成智能化操控体系,最终实现"以患者为中心"的精准个性化医疗.未来发展方向将聚焦于核心技术从辅助向半自主演进、智慧手术室构建与远程协同以及从疾病治疗向神经功能增强的拓展.该体系的建立与推广,将在技术创新与伦理规范的平衡中推动神经外科诊疗范式的重塑,为全球患者提供更精准、安全、高效的医疗服务.

The development of neurosurgery has now entered a new stage of Intelligent Neurosurgery,driven by artificial intelligence(AI)as its core force.Intelligent Neurosurgery,grounded in modern neurosurgery,integrates cutting-edge technologies such as AI,machine learning,surgical robotics,brain-computer interfaces(BCIs),big data,digital twins,and neuromodulation.Through a human-machine collaborative model,it constructs an intelligent diagnosis and treatment system covering the entire process of"precise preoperative evaluation—intraoperative real-time navigation and assistance—meticulous postoperative management."Its core technological pillars include deep learning-driven automated image segmentation and molecular subtype prediction,surgical robots achieving submillimeter precision,BCIs establishing direct communication pathways between the brain and external devices,multimodal data fusion integrating clinical,genomic,and imaging information to build predictive models for optimized decision-making,and digital twin and neuromodulation technologies enabling personalized treatment simulation and reversible neural regulation.Clinical applications now extensively cover maximal safe resection of brain tumors,personalized parameter optimization for functional disorders such as Parkinson's disease and epilepsy,and minimally invasive interventional treatments for cerebrovascular diseases.Furthermore,Intelligent Neurosurgery demonstrates significant value in military medicine for the rapid,intelligent management of traumatic brain injury.However,challenges including data privacy,algorithmic accountability,ethical norms,and uneven technology dissemination remain critical obstacles to its development.The essence of Intelligent Neurosurgery lies in empowering traditional disciplines with cutting-edge technologies to form an intelligent control system,ultimately achieving"patient-centered"precision medicine.Future directions will focus on the evolution of core technologies from assistance towards semi-autonomy,the construction of smart operating rooms and remote collaboration,and the expansion from disease treatment to neural function enhancement.The establishment and promotion of this system will reshape the paradigm of neurosurgical diagnosis and treatment by balancing technological innovation with ethical norms,ultimately delivering preciser,safer,and more efficient medical services to patients worldwide.
