阳虚质老年患者半髋关节置换术前血浆代谢物与术后谵妄相关性OA
Association between Preoperative Plasma Metabolites and Postoperative Delirium in Elderly Patients with Yang-Deficient Constitution Undergoing Hemiarthroplasty
目的 通过代谢组学从分子层面初步探索阳虚质(YDC)和术后谵妄(POD)的相关性.方法 选择拟行半髋关节置换术的老年患者于手术当日采集空腹静脉血,术后第1、3、5天采用混淆评估方法中文修订版(CAM-CR)量表动态评估患者的POD发生情况.以POD作为结局变量,通过单因素和多因素Logistic回归分析POD发生的危险因素.根据术前中医体质判定,分为YDC组(52例)和非YDC组(74例),采用非靶向代谢组学分析来筛选差异代谢物,并通过京都基因与基因组百科全书(KEGG)通路富集分析初步探索YDC老年患者发生POD的潜在机制.结果 Logistic回归分析结果表明,年龄(OR=1.11,95%CI:1.04~1.18,P<0.001)、体重指数(BMI)(OR=1.03,95%CI:1.01~1.06,P=0.041)及YDC(OR=1.40,95%CI:1.01~1.55,P=0.033)是POD的独立危险因素.非靶向代谢组学筛选出41种术前血浆差异代谢物,包括γ-氨基丁酸(GABA)、亚油酸、琥珀酸等.KEGG通路富集分析显示,差异代谢物显著富集于4条关键通路:亚油酸代谢通路(P<0.001)、过氧化物酶体增殖物激活受体(PPAR)信号通路(P=0.001)、环磷酸腺苷(cAMP)信号通路(P=0.003)和GABA能突触通路(P=0.005).结论 YDC是POD的独立危险因素.YDC老年患者发生POD的潜在机制与亚油酸代谢失衡、PPAR/cAMP信号异常及GABA能突触功能紊乱相关.
Objective To preliminarily explore the correlation between Yang deficiency constitution(YDC)and postoperative delirium(POD)at the molecular level through metabolomics.Methods Fasting venous blood samples were collected from elderly patients undergoing hemiarthroplasty on the day of surgery.POD was dynamically assessed using the Confusion Assessment Method-Chinese Revision(CAM-CR)scale on postoperative days 1,3,and 5.Using POD as the outcome variable,univariate and multivariate Logistic regression analyses were performed to identify risk factors for POD.Based on preoperative Chinese Medicine(CM)constitution assessment,patients were assigned into a YDC group(n=52)and a non-YDC group(n=74)for untargeted metabolomic analysis to screen differential metabolites.Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis was conducted to preliminarily explore the potential mechanisms underlying POD in elderly YDC patients.Results Logistic regression analysis showed that age(OR=1.11,95%CI:1.04~1.18,P<0.001),body mass index(BMI)(OR=1.03,95%CI:1.01~1.06,P=0.041),and YDC(OR=1.40,95%CI:1.01~1.55,P=0.033)were independent risk factors for POD.Untargeted metabolomics identified 41 preoperative plasma differential metabolites,including γ-aminobutyric acid(GABA),linoleic acid,and succinic acid.KEGG enrichment analysis revealed that these differential metabolites were significantly enriched in four key pathways:linoleic acid metabolism(P<0.001),peroxisome proliferator-activated receptor(PPAR)signaling pathway(P=0.001),cyclic adenosine monophosphate(cAMP)signaling pathway(P=0.003),and GABA ergic synapse(P=0.005).Conclusions YDC is an independent risk factor for POD.The potential mechanisms of POD in elderly YDC patients are related to imbalances in linoleic acid metabolism,abnormalities in PPAR/cAMP signaling,and disturbances in GABA ergic synaptic function.These findings provide a new direction for perioperative neuroprotective strategies based on CM constitution theory.
