基于1H-MRS观察杠杆定位手法调控腰椎间盘突出症即刻脑代谢效应OA
Immediate Cerebral Metabolic Effects of Lever Positioning Manipulation to Modulate Lumbar Disc Herniation:A 1H-MRS-based Study
目的 基于氢质子磁共振波谱(1H-MRS)技术分析单次杠杆定位手法(LPM)对腰椎间盘突出症(LDH)患者脑右侧楔前叶(PCu)代谢物的影响,探索LPM治疗LDH发挥即刻镇痛效应的脑化学物质基础.方法 34例为2023年9月—2024年5月在浙江中医药大学第三附属医院推拿科就诊LDH患者,25例符合纳入标准,简称LDH组.同时招募与LDH组性别、年龄相匹配的健康志愿者(简称HC组)25名.在LPM治疗前后,LDH组共进行两次1H-MRS扫描,并通过视觉模拟评分(VAS)和日本骨科协会(JOA)评分进行疼痛和腰椎功能障碍评估.HC组只与LDH组治疗前同一时间点进行1H-MRS扫描,根据前期本团队研究结果划定右侧PCu峰值坐标为感兴趣区(ROI),分析两组相同时间点及LDH组干预前后的N-乙酰天冬氨酸/肌酸(NAA/Cr)、胆碱/肌酸(Cho/Cr)、谷氨酸复合物1-6/肌酸(Glx1-6/Cr)等代谢物比值差异,并与VAS、JOA评分量表进行相关性分析.结果 与HC组比较,LDH组治疗前NAA/Cr、Cho/Cr值降低(P<0.05).与治疗前比较,LDH组治疗后NAA/Cr值、JOA评分升高(P<0.05);Glx1/Cr、Glx2/Cr、Glx6/Cr 值及 VAS 降低(P<0.05).LDH 组治疗前后 NAA/Cr 差值与 VAS、JOA 评分差值呈 负相关(r=-0.404,P<0.05;r=-0.440,P<0.05),Glx1 差值与JOA评分差值呈负相关(r=-0.504,P<0.05).结论 LPM可调控LDH患者右侧PCu脑区异常代谢水平,NAA和Glx1可能是LPM治疗LDH发挥即刻镇痛效应的关键脑代谢物质.(ClinicalTrials.gov No.NCT 05613179)
Objective To analyze the effect of a single lever positioning manipulation(LPM)on the metabolites in the right precuneus(PCu)of patients with lumbar disc herniation(LDH)using hydrogen proton magnetic resonance spectroscopy(1H-MRS),and to explore the brain chemical basis for the immediate analgesic effect of LPM in the treatment of LDH.Methods From September 2023 to May 2024,34 patients who visited the Traction Department of the Third Affiliated Hospital of Zhejiang Chinese Medical University and met the inclusion criteria for LDH were selected.The patients were assigned to the LDH group(25 cases)and the healthy control group(HC group,25 cases)in matched gender and age.Patients in the LDH group underwent two 1H-MRS scans before and after LPM treatment,and Visual Analogue Scale(VAS)and Japanese Orthopaedic Association(JOA)scores were used to assess pain and lumbar dysfunction.Patients in the HC group received no intervention and no clinical scale assessment,and underwent 1H-MRS scans at the same time point as the LDH group before treatment.Based on the previous research results of our team,the peak coordinates of the right PCu were defined as the region of interest(ROI)to analyze the differences in metabolite ratios such as N-acetylaspartate/creatine(NAA/Cr),choline/creatine(Cho/Cr),and glutamate complex 1-6/creatine(Glx1-6/Cr)at the same time points between the two groups and before and after intervention in the LDH group.Correlation analyses were conducted with VAS and JOA scores.Results Compared with the HC group,the NAA/Cr and Cho/Cr values in the PCu brain region of the LDH group before treatment were significantly lower(P<0.05).Compared with before treatment,the NAA/Cr value and JOA score in the LDH group after treatment significantly increased(P<0.05);the Glx1/Cr,Glx2/Cr,Glx6/Cr values and VAS score significantly decreased(P<0.05).The difference in NAA/Cr before and after treatment in the LDH group was negatively correlated with the difference in VAS and JOA scores(r=-0.404,P<0.05;r=-0.440,P<0.05),and the difference in Glx1 was negatively correlated with the difference in JOA scores(r=-0.504,P<0.05).Conclusion LPM regulated the abnormal metabolic levels in the right PCu brain region of LDH patients,and NAA and Glx1 might be the key brain metabolic substances for the immediate analgesic effect of LPM in the treatment of LDH.(ClinicalTrials.gov No.NCT 05613179)
