养心升阳法揿针对心阳不足证低度近视患儿眼部血流参数的影响OA
Effects of thumbtack needle therapy for Yangxin Shengyang on ocular blood flow parameters in treatment of low myopia in children with heart yang insufficiency
目的:观察养心升阳法揿针对心阳不足证低度近视患儿黄斑区脉络膜、视网膜厚度及血流的影响.方法:将 120 例心阳不足证低度近视患儿随机分为养心升阳法揿针组(观察 1 组,40 例,脱落 2 例)、眼周揿针组(观察 2 组,40 例,脱落 2 例)和对照组(40 例,脱落 4 例).对照组予散瞳验光及健康宣教;观察 1 组在对照组基础上予养心升阳法揿针治疗,穴取大椎及双侧阳白、攒竹、太阳、神门、心俞;观察2组在对照组基础上予眼周揿针治疗,穴取双侧阳白、攒竹、太阳.每3 d治疗1次,均连续治疗3个月.分别于治疗前后检测各组患儿黄斑中心凹脉络膜厚度(SFCT)、脉络膜毛细血管层(CC)血流密度、脉络膜中大血管层(CL)血流密度、黄斑区视网膜厚度(MRT)、浅层视网膜毛细血管丛(SCP)血流密度、深层视网膜毛细血管丛(DCP)血流密度、等效球镜(SE)、裸眼远视力、眼轴长度(AL),并于治疗前后评定各组患儿中医证候评分.结果:治疗后,观察 1 组SFCT、MRT及CC、CL、SCP、DCP血流密度均较治疗前增加(P<0.05),裸眼远视力较治疗前提高(P<0.05),中医证候评分较治疗前降低(P<0.05);观察2 组SFCT、CL血流密度较治疗前增加(P<0.05);对照组SFCT及CC、CL血流密度较治疗前降低(P<0.05),SE较治疗前进展(P<0.05).观察1 组SFCT、MRT、CL血流密度、DCP血流密度、SE、裸眼远视力、中医证候评分较治疗前改善优于观察 2 组及对照组(P<0.05).结论:养心升阳法揿针能够增加心阳不足证低度近视患儿黄斑区脉络膜及视网膜厚度,改善脉络膜及视网膜微循环,从而延缓屈光度进展,控制眼轴增长,并在一定程度上改善中医证候.
Objective To observe the effects of thumbtack needle therapy for Yangxin Shengyang(nourishing heart and elevating yang)on thickness and blood flow of macular choroid and retina in children with low myopia of heart yang insufficiency.Methods A total of 120 children with low myopia of heart yang insufficiency were randomized into an observation group 1(40 cases,2 cases dropped out),an observation group 2(40 cases,2 cases dropped out)and a control group(40 cases,4 cases dropped out).In the control group,mydriatic optometry and health education were provided.In the observation group 1,besides the interventions in the control group,the thumbtack needle therapy for Yangxin Shengyang was delivered at Dazhui(GV14),bilateral Yangbai(GB14),Cuanzhu(BL2),Taiyang(EX-HN5),Shenmen(HT7)and Xinshu(BL15).In the observation group 2,on the basis of the interventions as the control group,the thumbtack needle therapy was operated at periorbital acupoints,i.e.bilateral Yangbai(GB14),Cuanzhu(BL2)and Taiyang(EX-HN5).The intervention was given once every 3 days,for consecutive 3 months.Before and after treatment,the subfoveal choroidal thickness(SFCT),the blood flow density in the choroidal capillary layer(CC)and choroidal large vessel layer(CL),macular retinal thickness(MRT),and the blood flow density of superficial retinal capillary plexus(SCP)and deep retinal capillary plexus(DCP),spherical equivalent(SE),uncorrected distance visual acuity and eye axis length(AL)were measured in the three groups;and the traditional Chinese medicine(TCM)syndrome scores were evaluated.Results After treatment,SFCT,MRT,and the blood flow density of CC,CL,SCP and DCP increased(P<0.05),the uncorrected distance visual acuity was improved(P<0.05),and TCM syndrome score was reduced(P<0.05)when compared with those before treatment in the observation group 1.After treatment,in the observation group 2,SFCT and the blood flow density of CL were elevated in comparison with those before treatment(P<0.05).After treatment,in the control group,SFCT,the blood flow density of CC and CL were reduced(P<0.05)and SE was progressed(P<0.05)in comparison with those before treatment.In the observation group 1,the D-values of SFCT,MRT,the blood flow density of CL and DCP,SE,the uncorrected distance visual acuity and TCM syndrome score before and after treatment were larger than those in the observation group 2 and the control group(P<0.05).Conclusion Thumbtack needle therapy for nourishing heart and elevating yang increases the macular choroidal and retinal thickness and improves microcirculation in the choroid and retina in children with low myopia of heart yang insufficiency,so as to delay the progression of refractive errors,control axial growth,and alleviate TCM syndrome to a certain extent.
