应用单组目标值法评价眼针带针康复对卒中后患者认知功能及运动功能康复效果的研究OA
Evaluation of the effectiveness of periocular acupuncture rehabilitation therapy with needle retention for cognitive and motor function recovery in post-stroke patients using the single-group target value method
目的:采用单组目标值法评价眼针带针康复疗法促进卒中后患者认知功能和运动功能康复的效果.方法:前瞻性连续收集卒中后接受眼针带针康复的患者 102 例(眼针带针康复组),患者接受基础药物治疗及眼针带针康复,眼针穴取双侧上焦区、下焦区、肝区、肾区,每日1次,治疗5 d后休息2 d,共治疗 4周.分别于治疗前后及治疗结束后 12 周随访评价患者简易精神状态检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)、Fugl-Meyer评估量表(FMA)和改良Barthel指数(MBI)评分,并与传统针灸(针灸组)和常规康复(常规康复组)治疗效果的目标值进行比较,目标值来源于同类研究证据等级最高的Meta分析.结果:治疗前后各时点,眼针带针康复组患者MMSE、MoCA、FMA、MBI评分均存在时间效应(P<0.001,P<0.01),随时间延长评分分值逐渐升高.目标值法结果显示,在改善认知功能方面,治疗后,眼针带针康复组患者MMSE评分与针灸组目标值比较差异无统计学意义(P>0.05),MoCA 评分低于针灸组目标值(P<0.01);随访时,眼针带针康复组患者MMSE 评分高于针灸组目标值(P<0.01),MoCA 评分与针灸组目标值比较差异无统计学意义(P>0.05).在改善运动功能方面,治疗后,眼针带针康复组患者FMA和MBI评分均低于针灸组目标值(P<0.001);随访时,眼针带针康复组患者FMA评分与针灸组目标值比较差异无统计学意义(P>0.05),MBI评分高于针灸组目标值(P<0.05).治疗后和随访时,除治疗后FMA、MBI评分外,眼针带针康复组患者上述指标均优于常规康复组目标值(P<0.001).结论:眼针带针康复治疗在改善卒中后认知功能和运动功能方面与传统针灸疗效相当,优于常规康复疗法.
Objective To evaluate the effectiveness of periocular acupuncture rehabilitation therapy with needle retention(PARNR)for promoting the recovery of cognitive and motor functions in post-stroke patients using the single-group target value method.Methods A total of 102 post-stroke patients were included(periocular acupuncture rehabilitation group),using a prospective and consecutive collection,and treated with PARNR.Besides conventional medication,the patients received periocular acupuncture at bilateral upper-jiao area,lower-jiao area,liver area and kidney area,once daily,at interval of 2-day after consecutive 5-day intervention.The treatment for 4 weeks was required.Before and after treatment,and in follow-up 12 weeks after treatment completion,the scores of mini-mental state examination(MMSE),Montreal cognitive assessment scale(MoCA),Fugl-Meyer assessment scale(FMA)and modified Barthel index(MBI)were evaluated separately.The comparison was performed for the target values of the therapeutic effect between traditional acupuncture and moxibustion(acupuncture and moxibustion group)and conventional rehabilitation(conventional rehabilitation group).The target values were derived from the Meta-analysis with the highest evidence level in the same category.Results At each time point before and after treatment,the scores of MMSE,MoCA,FMA and MBI in the periocular acupuncture rehabilitation group showed the time effect(P<0.001,P<0.01),and the scores were increasing over time.Regarding the improvements in cognitive function,the target value method revealed that the MMSE score was not statistically different(P>0.05),the MoCA score was lower in the periocular acupuncture rehabilitation group after treatment(P<0.01);and the MMSE score was higher(P<0.01),and the MoCA score was not statistically different(P>0.05)in the follow-up,compared with the target values in the acupuncture and moxibustion group.For the improvements in motor function,when compared with the target values in the acupuncture and moxibustion group,the scores of FMA and MBI were lower after treatment(P<0.001);the FMA score was not different statistically(P>0.05)and the MBI score was higher(P<0.05)in the follow-up in the periocular acupuncture rehabilitation group.After treatment and in follow-up,except for the FMA,MBI scores after treatment,the above outcomes in the periocular acupuncture rehabilitation group were all superior to the target values in the conventional rehabilitation group(P<0.001).Conclusion The periocular acupuncture rehabilitation therapy with needle retention is comparable to traditional acupuncture and moxibustion in improving post-stroke cognitive and motor functions and superior to conventional rehabilitation therapy.
