体外膜氧合(ECMO)作为危重症患者的生命支持手段,虽然在挽救生命方面发挥了重要作用,但其应用过程中出血和血栓并发症的发生率较高,严重影响患者的预后.因此,如何有效评估出血风险并指导抗凝及输血管理成为临床面临的重要问题.黏弹性止血试验(VHA)作为一种能够实时监测血液凝血过程变化的技术,具有快速、全面和实时的优点,正在逐渐应用到ECMO患者的抗凝管理中.本文综述了VHA与传统实验室检测在出血及抗凝评估中的对比,并探讨其在ECMO抗凝管理中的应用价值,重点纳入近10年来旨在探讨VHA在ECMO抗凝管理中应用的临床研究文献,并全面评估了该技术在临床实践中的应用进展和未来发展方向,以期为ECMO患者的抗凝管理和输血决策提供理论依据和实践参考.
Extracorporeal membrane oxygenation(ECMO)serves as an essential life-support intervention for critically ill patients.Despite its life-saving potential,ECMO is associated with a considerable incidence of hemorrhagic and thrombotic complications,which significantly compromise patient prognosis.Consequently,the accurate assessment of bleeding risk and precise management of anticoagulation and transfusion therapy have emerged as paramount clinical priorities.Viscoelastic hemostatic assay(VHA),which provides a dynamic,real-time profile of the hemostatic process,is being increasingly integrated into the anticoagulation management of ECMO patients due to its rapidity and comprehensive nature.This review systematically evaluates the correlation between VHA and conventional laboratory assays in the context of bleeding and anticoagulation monitoring,and critically appraises the evolving role of VHA in guiding ECMO anticoagulation.Focusing on clinical studies from the past 10 years,this review comprehensively evaluates the current applications and future directions of this technology in clinical practice,with the aim of providing a theoretical basis and practical guidance for anticoagulation management and transfusion strategies in ECMO patients.
