布比卡因脂质体用于臂丛神经阻滞联合全麻对肩关节镜手术患者的术后镇痛效果及安全性OA
Efficacy And safety of Bupivacaine Liposome in Brachial Plexus Block Combined with General Anesthesia for Postoperative Analgesia in Patients Undergoing Shoulder Arthroscopy
目的 探讨布比卡因脂质体用于臂丛神经阻滞联合全麻对肩关节镜手术患者的术后镇痛效果及安全性.方法 选择2024年1月至2025年6月丹阳市妇幼保健院收治的肩关节镜手术患者62例,以随机数字表法分组,对照组(31例)予0.375%罗派卡因臂丛神经阻滞联合全麻,观察组(31例)予布比卡因脂质体臂丛神经阻滞联合全麻.比较两组术后镇痛泵按压次数、补救性镇痛次数,评估术后各时间点的疼痛程度,统计两组不良反应.结果 观察组术后镇痛泵按压次数与补救性镇痛次数均少于对照组(P<0.05).观察组术后6 h、12 h、24 h、48 h、72 h的NRS分值均低于对照组(P<0.05).观察组不良反应率低于对照组(P<0.05).结论 在肩关节镜手术中应用布比卡因脂质体臂丛神经阻滞联合全麻,可提高术后镇痛效果,减少术后补救性镇痛措施的使用,并减少不良反应.
Objective:To investigate the postoperative analgesic effect and safety of bupivacaine liposome in brachial plexus block combined with general anesthesia for patients undergoing shoulder arthroscopy.Methods:From Janu-ary 2024 to June 2025,62 patients who underwent arthroscopic shoulder surgery in Danyang Maternal and Child Health Hospital were randomly divided into two groups.The control group(31 cases)was given 0.375%ropiva-caine combined with general anesthesia,and the observation group(31 cases)was given bupivacaine liposome com-bined with general anesthesia.The times of pressing analgesia pump and remedial analgesia were compared be-tween the two groups,and the degree of pain at each time point after operation was evaluated,and the adverse re-actions of the two groups were counted.Results:The times of pressing analgesia pump and remedial analgesia in the observation group were less than those in the control group(P<0.05).The NRS scores of the observation group at 6 h、12 h、24 h、48 h and 72 h after operation were lower than those of the control group(P<0.05).The adverse re-action rate of the observation group was lower than that of the control group(P<0.05).Conclusion:Bupivacaine li-posome brachial plexus block combined with general anesthesia can improve postoperative analgesia effect,reduce the use of postoperative remedial analgesia measures and reduce adverse reactions.
